He doesn't suggest we deceive our patients but to explain we don't know, he feels, detracts a patient's trust and confidence in their clinician.

Sincere condolences to the Donelson family. Much gratitude for all that Dr. Donelson did to advocate for patients. As a frequent guest on the podcast and our sister podcast, Inside the Science, he will be missed but we are all better because of his contribution and we now have the opportunity to stand on his shoulders to move upward and onward.

It gives us great sadness to inform you of the death of Dr. Ronald G. Donelson on the 27th of December 2023.

Ron was one of the original members of the McKenzie Institute and his contribution to its development was profound. His paper on centralisation, published in Spine in 1990, paved the way for further research into the McKenzie approach. In addition to research, Ron taught physician courses for many years and was a tireless advocate for MDT in the medical community. If any MII faculty ever needed help with clinical writing or research Ron was always there for them; He will be greatly missed.

Sciatica is very common. Epidural injections are often recommended but are they helpful? This systematic review comes away with important data on the reality of their effectiveness. Slight and temporary improvement seems to be the consensus when broadly viewed. Like with many results, this may suggest the key is shared decision making with the right patient at the right time. The A-D-T-O model was proposed over 20 years ago and still, so many studies are done to learn if a treatment is effective when the appropriate work has not been done to establish if the subjects in the study have been properly diagnosed(are homogenous) and if the assessment methods used to diagnose are even reliable and valid. The words we use with patients matter.

The labels clinicians give patients influence their outcome.

This has been shown with lumbar and shoulder diagnoses.

However, just as we do with graded exposure of movement, what we say to patients should change and be delivered to the right patient at the right time.

Perhaps our words should be different at one phase of their recovery compared to another.

Whereas certain words may elicit fear at one point, the same words may inform and create resilience at another.

When we understand a capable musculoskeletal system enabling regular movement:
- is able to reduce disability as a result of osteoarthritis by 47%,
- can control blood pressure and cholesterol,
- reduces the rates of the progression of dementia and alzheimers by 50%,
- in combination with other lifestyle changes, reduces the progression of diabetes by 58%,
- is the number one treatment to reduce fatigue, and
- improves mental health by reducing depression and anxiety
When we understand this - that a healthy, capable musculoskeletal system which allows full and free movement has a huge affect on all other body systems... perhaps our greatest impact in helping patients with pain and limited movement is MUCH more than just helping them move without pain... change my mind!

The pareto principle suggests 80% of the consequences come from 20% of the cause in any situation. So, whether it's relief of pain or recovering one's ability to walk or sleep or reach overhead, could it be that the majority or what is done to patients with these problems has little impact on solving the problem?

Knowing what we know about comparative effectiveness studies and many interventions having no greater results than placebo, I'd say this may be a safe bet.

How would you weigh in on this?

Are CPRs really "rules"? Check out this evidence on our sister podcast!

CPRs were all the talk several years ago and are still utilized to select patients to receive specific treatment but more research like Schenk 2012 in the JMMT "Effectiveness of mechanical diagnosis and therapy in patients with back pain who meet a clinical prediction rule for spinal manipulation" suggests that those who fall within the "rule" can be equally managed through other treatment approaches. In this paper Schenk showed MDT equally effective for those eligible for spinal thrust manipulation STM per the STM CPR.
