📌Featured Journal for Black History Month: Inefficiency in Delivery of General Surgery to Black Patients: A National Inpatient Sample Study:

📃🏥 Racial disparities in outcomes among patients in the United States are widely recognized, but disparities in treatment are less commonly understood. This study is intended to identify treatment disparities in delivery of surgery and time to surgery for diagnoses managed by general surgeons—appendicitis, cholecystitis, gallstone pancreatitis, abdominal wall hernias, intestinal obstructions, and viscus perforations.

💬 Results:
◼️ Black patients were less likely to receive surgery for appendicitis, cholecystitis, pancreatitis, and hernias, and more likely to receive surgery for obstructions and perforations.

◼️ Black patients experienced longer wait times prior to surgery, by 0.15 to 1.9 days, depending on the diagnosis.

◼️ Mediation analysis demonstrated that these disparities are not attributable to the patient factors available in the NIS, and provided some insight into potential contributors to the observed disparities, such as hospital factors and socioeconomic factors.

🏆 VCOT Most Outstanding Resident Paper Award 2024: Thieme congratulates veterinarian Alecia Evans (Murdoch University Perth)! Together with her co-authors Mark Glyde, Rob Day and Giselle Hosgood, she has investigated biomechanical parameters that influence the stability of implants used to treat bone fractures in animal patients.

💡 Using twisting and bending tests, she was able to show how the screw position of an implant affects its stability as well as evaluate the appropriate distance between the plate and the bone. If there is a large gap between screws on either side of the fracture, the distance between the implant and the bone should be as small as possible in order to withstand greater loads.

The research results help surgeons to optimally select and place implants, which can reduce possible complications and allow optimal healing of bone fractures.

Alicia Evans' study was published in 2023 in the Thieme journal VCOT - Veterinary and Comparative Orthopaedics and Traumatology. The work was part of her specialist training at Murdoch University in Perth, Australia. The veterinarian now works as a surgeon in an emergency clinic for animals in Brisbane.

With the VCOT Most Outstanding Resident Paper Award, the editors recognize outstanding experimental or clinical studies that veterinarians have published in the VCOT as part of their "residency". When making its decision, the jury attaches great importance to the fact that the work is based on a subject-specific problem and a solution for it. Soft Tissue Coverage of the Posterior Cervical Spine 👇

The ideal flap for coverage of the posterior cervical spine should be local, pedicled, low morbidity, and capable of reaching to the occiput. First-line local options include the trapezius, latissimus dorsi, and paraspinous muscle flaps.

One of the most reliable and robust options for flap coverage of the cervical spine, including the posterior scalp, is the trapezius muscle. Wounds over the cervicothoracic spine are susceptible to mechanical stress such as pressure and tension, making the proximity, size, and large arc of rotation offered by the trapezius a highly favored selection. The trapezius can be rotated or advanced, but special care must be placed in elevating the superficial muscle off those that lie deep.
