🩺Case: A mother brings her 6-year-old daughter to her pediatrician with symptoms of a sore throat and red, tearing eyes. The patient complains of itchy eyes when she woke up that morning. Her symptoms started the previous day, and she missed school today.

History reveals that the child’s vaccinations are up to date and that she has been previously healthy with no prior hospitalizations. Upon examination, the patient is found to have a temperature of 39°C, blood pressure of 90/70 mmHg and respiratory rate of 20 breaths/min.

She has upper lid edema with slight subconjunctival hemorrhage. Her mother mentions that other classmates were also out sick during the past week with similar symptoms.

📘Von Willebrand disease (VWD) is a hereditary coagulopathy that arises from a deficiency in the quantity or quality of Von Willebrand factor (VWF).

🩸VWF is produced by megakaryocytes and endothelial cells. It is found in plasma, platelets, endothelial cells, and subendothelial cells.

🩸 In cases of VWF deficiency (quantitative or qualitative), both primary hemostasis (impaired platelet adhesion) and secondary hemostasis (fibrin deposition via coagulation cascade) are impaired.

Anatomy for the facial injector is unique and complex.

📌Injectors should consider the superficial musculoaponeurotic system (SMAS) as a depth gauge to assist in navigating from superficial to deep and vice versa during clinical injection.

💡Simplistically, the face has five concentric layers.
Layer 1 is the facial skeleton and periosteum.
Layer 2 is the “sub-SMAS” plane. It contains deep fat compartments, potential spaces, and the origins of the “true” osseocutaneous retaining ligaments.
Layer 3 is the SMAS layer and layer 4 is the “supra-SMAS” layer. It contains the superficial fat compartments and the retinacular cutis extension of the “true” osseocutaneous retaining ligaments.
Layer 5 is the skin.

If the diameter of the aorta is up to 30 mm, it is called ectasia. There are many definitions of an arterial aneurysm. A general definition is an increase in diameter of at least 50% compared to the normal diameter of that artery.

📌Traumatic intracranial aneurysms (TIAs) are rare vascular lesions that represent < 1% of all intracranial aneurysms. They most commonly occur in males in the third decade of life, and are associated with high morbidity and mortality. They may occur following either blunt trauma (90% of cases) or penetrating trauma (10% of cases).

📌Most TIAs are false aneurysms, precluding direct microsurgical repair or endovascular obliteration. Consequently, aneurysm trapping or parent artery occlusion is often required, with consideration of arterial bypass when the risk of resultant ischemic injury is high.

📌Endovascular techniques have developed as a viable alternative when clinical status precludes safe operative intervention.
