No reason too ARGUE with a person with dementia. I use my approach for anyone with memory issues CHEER. Calm,Help,Embrace,Empower,Reminisce. They feel unnerved so you being thr calm in their life is.priceless. u are there to help. Embrace the person before you not who you remember.Empower them to be involved whre they can. They had a life if independence. Reminisce the old memories is what they have left and even they are leaving.

Observation Service
Observation services are often provided in combination with outpatient emergency or surgical services. The observation unit affords patients a place for further evaluation that may be necessary before a final determination is made about care.

Patients in observation status enjoy many of the amenities offered to inpatients. These include placement in a hospital room, access to television, phone service and in-room dining. Observation patients may stay in the hospital overnight, but observation is actually an outpatient service.

In most cases, a decision will be made within 24 hours whether to admit a patient to inpatient status or discharge them home to follow up at their doctor’s office.

This means even if they keep you overnight your are not seen as admitted thus the bill is more of your responsibility. Be sure that the words you are admitted are said before continuing in any after care after this one night of observation. Read your medicare handbook we realize it is huge!!!

Acknowledgement of the Roles and Responsibilities for Receiving
Consumer Directed Personal Assistance Services
Responsibilities of the Health Plan (PDF, 83KB)
I acknowledge that Consumer Directed Personal Assistance Services (CDPAS) allows chronically ill and/or physically disabled members receiving home care services greater flexibility and freedom of choice in obtaining such services.

In order to receive CDPAS I understand the respective roles and responsibilities of the consumer and the ________________________________________ Health Plan (Health Plan).

RESPONSIBILITIES OF THE HEALTH PLAN
The Health Plan will:

Determine whether the member is on the most current Plan Roster.
Provide the member requesting personal care services with information about how to qualify for CDPAS and other community based long term care services.
Provide the member with written educational materials outlining the roles and responsibilities for the member/designated representative if member expresses an interest in CDPAS.
Assess whether the member is eligible to receive home care or personal care services.
Determine if the member is able and willing to assume all responsibilities associated with receiving the service, or has a designated representative or other identified adult, able and willing to act on the member´s behalf.
Determine whether member is eligible to receive CDPAS.
Assess and document the member´s health patient centered care plan to assure adequate supports are available to meet the member´s needs.
Authorize the type, amount and level of services required by the member.
Develop a plan of care with the member, outlining the tasks to be completed by the personal assistant. The plan of care document will be maintained with Plan and a copy will be provided to the member.
If it is determined that the member is no longer eligible to continue receiving CDPAS, or Plan terminates the member´s receipt of CDPAS the MCO will assess on an ongoing basis whether the member requires personal care, home health care or some other level of service.
Provide the member with appropriate notices including a notice of fair hearing for reduction, termination of the level and amount of services or determining that the member is not eligible or no longer eligible to receive CDPAS.
RESPONSIBILILTIES OF THE MEMBER
The Member/Designated Representative (Member) will:

1. Review the information provided by the Plan about CDPAS and understand the roles and responsibilities of the Plan, the fiscal intermediary and the Member.
2. Be responsible for recruiting, hiring, and training, supervising, scheduling and terminating the personal assistant(s) of the member´s choosing in adequate numbers to meet the needs of the member.
3. Maintain an appropriate home environment for the safe delivery of care required by the member.
4. Train the personal assistant(s) to implement the plan of care.
5. Comply with labor laws, providing equal employment opportunities as specified in the agreement between Member and the Fiscal Intermediary (FI).
6. Inform the Plan and the FI of any change in status or condition including but not limited to: hospitalizations, address and telephone number changes, vacations within 5 business days.
7. Assure the accurate and timely submission of the personal assistant´s required paper work to the FI including time sheets, annual worker health assessments, and required employment documents.
8. Develop and maintain a contingency plan to assure adequate supports are available to meet the member´s needs.
9. Review and sign the personal assistant´s timecards assuring that the hours reflect the actual number of hours worked within the weekly authorized hours.
10. Cooperate with the Plan and agree to comply with Medicaid Managed Care Program requirements including but not limited to availability for required reassessments.
11. Report and return to MCO any overpayment or inappropriate payments from the Medicaid program made to Consumer Directed Personal Assistants.

Sundowner and wandering are just a person with Dementia's searching for purpose. Help them find their purpose.

Example:
Elderly woman spotted by my husband and I walking barefoot with no coat on Saturday in Pottersville NY. We were going to eat at a favorite restaurant. I asked staff if they knew the woman. They said they did.Her family took her shoes to keep her from wandering.

Watching her on her journey she had a purpose. She worked in this area all her life. She tended the flowers outside the restaurant and returned home.

The family was notified and apparently the police were aware of the situation as well.

Met a couple at a Dr. appt today. They were have difficulty ambulating and were feeling getting old was seen as a bad thing. We talked for a bit before my appt. I wished them the best.

Afterwards the husband was sitting waiting for his wife and we spoke again. This man had legitimate reasons for feeling overwhelmed and frustrated. He felt that he wasn't being listened to and that he was losing his dignity.

His wife needed more help and had more appts. yet the children didn't want him to drive. He was feeling his life was being taken by him by his children not his aging process.

The world is changing fast. Imagine how it effects your loved ones who were born so long ago. Don't escalate just to be right. Be kind. Be mindful of your words. They need help.

My dad gave me this advice a long time ago when I moved into my first apartment. He even came out and changed the screws for me as soon as I moved in. Someone else recently posted about it and I thought I would share!
A good home security tip that you may have never thought about.... Most contractors install the plates with the supplied screws which are only a half inch long and come out with one kick by a burglar. He installed 4 inch screws in their place that go through the door frame and into the framing of the house. They can kick for a long time before they get tired! You can see in the picture the screw in his hand is the screw supplied with the door hardware. Making a burglar make a bunch of noise and be foiled in their initial plan can not only give you time to arm yourself but they will most likely move on to an easier target.

Shampoo by the sink is a sign that your loved one maybe washing their hair by the sink and not actually bathing. Clean hair is great. However, this can not replace, the other needs that are met while showering or bathing. Don't accuse if you think assistance is needed. Gently have an adult conversation about needs. Provide simple help at first to assess. Then make a plan for yourself how to help and take care of what the true needs are.

Remember to take the CHEER skills with you as you proceed.......Calm, Help, Embrace, Empower and Reminisce. You need to be Calm (for your loved one needs help and may feel embarrassed), You need to be Helpful (can be seen as weakness by your loved one they need HELP who says?). You need to Embrace the person in front of you (not who they used to be). You need to Empower your loved one so they feel like they have purpose ( remembering they had a whole life without needing help such as this). You need to Reminisce to hear their stories and to share yours (if this is too hard for anyone involved remember humor always works).

SCAMMERS

I write this out of experience. Scammers can get your money in many ways. My advice today is to check your credit reports yearly minimally. Most banks do this yearly for free.

If you think someone has your information call TransUnion and have a password intiated. They will put on each report. The credit bureaus are TransUnion, Equifax and Experian.

Need help or clarification message or call.

As a child who cared for her parents with Alzheimer's I needed to realize there came a point that I was the adult and I had to make the decisions. And due to my parents condition I was not going to look for them for permission. If you ever find yourself in this position say a little mantra below.

Say this little mantra in your head "I no longer need my mother's permission to take care of my mother's needs"

Pose this question to your local politicians.

How are we protecting our seniors from fraud and scammer phone calls?

Received several calls in 2 days from 634/639 numbers that are defunct. I am screening phone calls for my friends' mom. They are requesting personal information and of course wanting money.

When you dont provide what they want the disgusting language is unreal. These seem to coincide with warranty calls.

They apparently are using voice recognition and have a way of stringing one call to another. They slam social media (as they should) what about slamming the phone companies. They have no responsibility in monitoring these land-line numbers?

If this was not monitored the woman I help would be in a heap of trouble in her late 80's.
And would be unable to undo the damage. How do we fix this gentlemen?
