Are you a lean mass hyper-responder? Would you bet your life on it?

This 1-year study is certainly interesting as they have enrolled both patients with known coronary artery disease and those without coronary artery disease (verified by CCTA).

However, factors that separate this population from the general public are most notably
-adherance to a ketogenic diet for >2 years
-HDL >60
-triglycerides

This podcast episode concentrates on cancer screening, detection, and prevention. Link in bio and story!

We go into detail about the benefits and detriments of screening tests (high sensitivity, low cost), early detection tests (often high cost, high chance incidental finding, but can find cancers that are otherwise not screened for).

Prevention: we discuss modifiable risk factors and which are attributed to the most cancers. Some of these include obesity, to***co, and alcohol. We also discuss smokeless to***co and va**ng.

Prevention: we discuss family history, and hereditary cancer genetic screening (we use Invitae but occasionally dig through full genome sequencing. Polygenic risk scores are not a validated tool for clinical decisions!)

Detection: we discuss screening including from USPTF, ACS (American cancer society), and there differences from other society recommendations.

We also discuss cutting edge tests like ISO-PSA, pirads and Gleason scores, and cancers that cannot be screened for but can be early detected with tests like MRI (including prenuvo and ezra), and the only epigenetic test we order routinely for the right patient (Galleri test, known as Grail liquid biopsy). Yes, we discuss the downsides of these tests as well. The patient needs to know the cons before we order the test.

Please comment below with your experiences and recent successes or struggles with cancer prevention and screening.

In my last podcast, I discuss how to optimize your s*xual health. Similar to other areas of health lifestyle is most important. It is reasonable to want optimal s*xual performance, just as it is reasonable to desire, optimal, athletic and cognitive performance.

Like other areas of health, the most powerful interventions are lifestyle changes. They aren’t as s*xy as medications and supplement (pun intended).

Improving exercise is your best first step (120mins zone 2, 2+ days a week of vigorous cardio, and 3+ days a week resistance training). This can lead to improved blood flow, improved core and pelvic muscle strength, and avoid metabolic syndrome and it’s downstream effects.

Diet and supplementation can also be emphasized to contain adequate nitric precursors (for example L-citrulline or other foods with known vasodilator benefit like beet root or garlic).

Stress should be under control in order to balance the “teeter totter/SeeSaw” of fight or flight vs rest and digest. This also includes social and relationship stressors, which are often most important in s*xual health.

Speak with your physician or NP about medications which can help. Balancing dopamine and serotonin is important to avoid premature or delayed or**sm\ejaculation in males and females. APO and even bupropion when indicated can help prevent delayed or**sm and abnormally long refractory periods. PDE5 inhibitors like Tadalafil can be a good addition, especially in older ages where pde5 levels rise naturally. Dose should be the minimally effective dose, usually 2.5mg.

Todays podcast is a deep dive into omega supplementation.

Full podcast link in bio and story.

We address many topics including:

Rancid supplements, optimal form, optimal ratio (different for each individual), Rx vs supplement form, triglyceride vs ethyl ester form, and various benefits.

Have you also noticed an increased incidence of afib anecdotally? What are some of the causes you think contribute? Could high dose EPA without DHA/DPA/ALA be one? Spoiler: the evidence is still mixed but for those at risk please consider your ratio.

Just for fun here is my “DysFunctional med afib stack”: our hypothetical patient is a 75 year old with history of “sluggish thyroid”, “chronic fatigue”, and “adrenal fatigue”. Patient is on daily hydrocortisone “replacement”, armour thyroid 2 grains, modafinil 200mg, pure EPA gems (2g BID because “doesn’t want to take statin”), and was recently given testosterone pellet (unknown dose) and Albuterol inhaler. Patient complains of fluttering in chest and fast heartbeat.

Testosterone levels vary, especially at lower SHBG levels (younger males tend to have lower SHBG). On replacement TRT, even at a level that looks “mid-normal”, the “area under the curve” is greater.

There are exceptions to this, like mostly lymphatically absorbed testosterone “Tlando” and nasal testosterone “Natesto”. Tlando chart is on last slide.

Yes, if your doctor recommends, some patients do use basal subq TRT + bolus morning Tlando or natesto. However, for most there is little upside to doing this.

Most free Testosterone assays are immunoassays, and are not very accurate. You are generally better off calculated your free T (use total T+SHBG).

Why are the same treatments (in traditional medicine) used for s*x offenders and women with PMDD?

Find out in the full podcast where we discuss chemical castration, anti-androgens, SSRI use, and more in the context of PMDD.

PMDD (premenstrual dysphoric disorder) is a common condition that causes a range of both physical and mental symptoms typically in the luteal phase of a woman's menstrual cycle.
A few examples below:

Physical symptoms: fluid retention, breast tenderness, cramping, decreased coordination, body aches
Mental symptoms: anxiety, irritability, depression, food cravings, mood swings

Lifestyle has the capacity to lead to significant improvement as well as significant worsening of symptoms:

For example, alcohol use and poor stress management can certainly exacerbate symptoms in the luteal phase.

The mental symptoms associated with PMDD seem to be tied to progesterone's downstream effect on the GABA receptor via allopregnanolone and seem to depend on an individuals sensitivity to the increase or withdrawal of this hormone.

This can be accomplished in a number of different ways such as stabilizing progesterone or reducing the 5 alpha reduced byproducts of progesterone, allopregnanolone and isoallopregnanolone. (This of these like T3 and reverse T3, the ratio determines the agonist vs antagonist effect).

Fluctuating allopregnanolone levels = symptoms
High allopregnanolone levels with minimal fluctuation = less symptoms
Low allopregnanolone levels with minimal fluctuation = less symptoms
Stable ratio of isoallopregnanolone: allopregnanolone = less symptoms

What is Individualized Medicine?

Gillett Health's Individualized Medicine combines each individual's unique health goals and/or medical conditions with evidence-based interventions that match each person's resources and values.

Evidenced-Based Interventions
-lifestyle changes
-science-backed supplementation (hint most supplement too often, and most are not evidence based)
-medications when indicated (benefit > risk, shared decision making)
-therapy/CBT

We believe in supporting one's health primarily through the 6 (now 7) pillars of health.
-Diet
-Exercise
-Sleep
-Stress
-Sunlight
-Spirit
-Social

Resources and Values
-each person has a different amount of free time, finances, baseline health, beliefs, and goals. It is important to meet each person where they are and build a plan together around the aspects of health that are most important to that individual.

Health Goals and Medical Conditions
-regardless of whether an individual has multiple medical conditions or has never had a health issue before, our goal is to empower individuals to reach their health goals. Rather than "I am sick and need to change this", we strive to intervene and create a positive mindset to think "I am healthy and want to change to improve my quality of life and healthspan".

Are you at a high BMI, but at a health body composition like , or are you at a normal BMI, but an unhealthy body fat percentage, like I was after my first son was born? For those under a BMI of about 30, body composition (DEXA scan is my favorite) is a much better measure of health than BMI. The scale is just a tool that can be deceiving and helpful. How has the scale helped or hurt you in the past?

Please listen to full podcast (link in bio and story) for specific information regarding definition of a healthy body composition, reasonable goals, and the surprisingly small amount of diet and exercise required to positively alter body composition (As long as you are consistent!).

Hair Loss Update Podcast 👨‍🦲

Kyle Gillett, MD and James O'Hara, NP review less common, but currently available treatments for androgenic alopecia (male pattern baldness).

Two currently available topical treatments (CosmeRNA & Clascosterone) as well as topical treatments in development center around blocking/degrading the androgen receptor. In fact, one of these treatments is described as delivering a "warhead" to the androgen receptor in the clinical research.

A monoclonal antibody targeting prolactin is in development (same drug also in development for endometriosis) for androgenic alopecia as well and a phase I trial participant has shared his (alleged) experience online.

With many other diseases utilizing multiple drugs/interventions leading to better outcomes, hair loss seems to be following a similar pattern.

Let us know your current hair growth protocol in the comments below 👇

Similar to measuring your weight on a scale, CGM have a lot of benefits and potential detriments.

Our latest podcast discusses a lot of the evidence for and against CGM use outside of diabetes. We discuss what conversations you should be having with your interdisciplinary team (for example our sports dietitian, or our preventive medicine physician).

We discuss MARD data, when to calibrate (or not), and examples of recommendations which can be harmful.

Instead of becoming dogmatic or emotional about CGM use, understand that it is simply an objective tool. In most cases, without diabetes, they are best utilized for short periods of time. In many cases the benefit is minimal enough they shouldn’t be utilized at all. However, for the 90% of Americans that have suboptimal metabolic health, they can be very useful. We like to use them along with consultation with a dietitian and DEXA scans to track lifestyle progress faster than an A1C can, in real time.

What negative or positive experiences have you had with a CGM?

To learn more: Please tune into the podcast, which is now posted on all platforms.

Ever seen the movie Wall-E? As I watched it, I was amazed the crew could walk after years of deconditioning, even if they were on TRT. A small amount of activity helps maintain strength required for a healthspan.

It is common for doctors to gain a lot of body fat during residency when they are doing long shifts, especially shifts that do not require moving. I struggled with this too, and now it’s second nature so my nickname is “walking enthusiast”. Over time, we develop habits. A worthy goal is to be consistent with mostly healthy habits to avoid feeling trapped counting calories (or stuck on a list of meds) indefinitely.

In other news, Wall-E was a great movie, and gardening and growing your own food is an efficient hobby for physical and mental health.

What is your favorite lifestyle habit? What are you working on now?

If you had only one plant to grow, and it would cover the whole earth, (no animals exist except cockroaches), what would it be? Peas? Olives? Potatoes? Quinoa? Soy beans? 🧐😉

How can we improve public health on a large scale? Primarily by providing the public with actionable tools, which are evidence based, to improve lifestyle. This happens before medication is necessary.

Just like preventive maintenance on your car should be done before adding nitrous and a turbocharger, True preventive medicine is the cornerstone of any health optimization goal.

My guest is who is a physician and data expert. He holds an MPH with emphases on statistics and population health informatics. He completed his preventive medicine residency at and is a dear friend and colleague at , our clinic.

We discuss some of the nuances with the various clinical trials for both semaglutide and tirzepatide. We also briefly discuss statins and how to interpret poor data and good data (this previously discussed more in depth in prior episode). Finally, we emphasize the interdisciplinary team and how scientists and clinicians can work together for optimal patient centered care, and reach an optimal outcome for those that may not meet a criteria for an “evidence-based algorithm”.

Luxurious all-inclusive vacations? Millions of dollars in speaking and consulting fees paid to physicians? Marketing drugs for off-label use?

The pharmaceutical industry certainly has a troubled past with all of the above occurring and expensive penalties and settlements as a result.

Some states have sought to limit pharmaceutical influence in medicine by setting a $100/year limit (including food) to the total amount of compensation to healthcare providers. We discuss the positives and the negatives of regulations like this.

We discuss the stages of clinical trials and what it actually takes to get a drug candidate FDA approved and the monitoring that happens even after a drug is approved.

We also take the time to answer some of the viewers' questions about the pharmaceutical industry including, "How do you know if your doctor is compromised by big pharma?"

"What solutions do you propose for how pharma can benefit patients and health care providers the most? Let us know in comments below.”
