UPDATE: CORONER ID'S ACCIDENT VICTIM AS HAVANA WOMAN
Fulton County Coroner Austin Rhodes has released the name of the individual that died after a two-vehicle collision involving a passenger car and commercial vehicle on US Route 136, near Bottom Lane in Lewistown. Melissa Phelps, 42, of Havana, was the driver of the passenger vehicle and was pronounced deceased at the scene. This incident remains under investigation by the Fulton County Coroner’s Office, Illinois State Police and Fulton County Sheriff's Office.

RELEASE: FATAL ACCIDENT ON ROUTE 136
In the afternoon hours on 5-19-25, the Fulton County Coroner Austin Rhodes can confirm that one person has died after a two-vehicle collision involving a passenger car and commercial vehicle on US Route 136, near Bottom Lane in Lewistown. The name of the deceased is being withheld and will be released later, pending next of kin notifications. An autopsy will be scheduled, and toxicology testing will be performed per Illinois state statute. This incident remains under investigation by the Fulton County Coroner’s Office, Illinois State Police and Fulton County Sheriff's Office. Also assisting at the scene was the Fulton County Emergency Medical Association, Lewistown Fire Department, Havana Rural Fire Department, Havana Police Department and Illinois Department of Transportation.
