WE'RE ALMOST THERE SUBSCRIBERS!

Mark your calendars this weekend (March 18-19, 2023) as we will be releasing your Hard Copies and Photo Frames. We will be releasing at the OSA Hall in the Office of Student Affairs (OSA).

To claim your frames, please bring your School ID, and your Blazer Official Receipt (either hard copy or soft copy/photo). If your receipt has been misplaced or if you have not been given your OR numbers by the staff via Messenger, you may show us a photo of your clearance signifying you are fully paid. This does not apply to those who are not fully paid but have received a clearance with a promissory note.

We will be having our masterlist as a reference whether or not you are actually fully paid. But to speed up the process, bring your receipts!

Off-campus due to OJT? Don't worry! You may have an authorized person claim your frames and photos for you! Just let them bring the following:

1) Authorization Letter that has your school ID or ID number, full name, and college of the subscriber; signifying that you are allowing then to claim your photos, and that they will be responsible for any damages once released; and
2) Official Receipt, either hard copy or soft copy.

If you have any questions about your photos, you may approach the Lights and Lenses staff that will be with us upon release!

ATTENTION SUBSCRIBERS!

The deadline of all payments and accountabilities will be on or before FEBRUARY 10, 2023 (Friday).

There will no longer be extensions as we will be finalizing the final count of subscribers. Failure to settle remaining accountabilities will result to revoking the subscriptions of subscribers. This means that you will not be able to claim your photos (hard and soft copy), frames, and yearbooks. You will also NOT be refunded as the payments are NON-REFUNDABLE.

If you are not fully paid with the Blazer Subscription, you are REQUIRED to answer and fill out the Google Form provided below, on or before FEBRUARY 6, 2023.

This will serve as your confirmation whether you wish to continue your Blazer Subscription or not. Please intently read the entirety of the form as there are information crucial to your unsubscription, i.e., still needing to pay the remaining payment for the photography of this year's Blazer project and half of the operational expenses. You may refer to the subscription breakdown to past posts and announcements in the page.

Failure to respond to this form will automatically revoke or terminate your subscription. Deadline of all payments and accountabilities are on February 10, 2023.

As per the Blazer terms and conditions, those who fail to pay the first installment this semester will not be included in the final listing, and will not appear in the yearbook.
