During an inspection on a vessel cold lay-up, the lifejackets stored underneath a sitting bench in the wheelhouse were found burned and melted.
A Lithium battery had exploded in one of the self-igniting lights.
As the bench was in a properly closed position, without any gaps, the fire could not spread due to a lack of oxygen.
Therefore the fire was not detected and did not activate a fire alarm, as the smoke was contained inside the storage compartment.
Investigation showed that the battery of the โLalizas 71209โ life jacket light had ๐ฒ๐
๐ฝ๐ถ๐ฟ๐ฒ๐ฑ ๐ณ๐ถ๐๐ฒ ๐บ๐ผ๐ป๐๐ต๐ ๐ฒ๐ฎ๐ฟ๐น๐ถ๐ฒ๐ฟ.
A possible cause could be that this battery was exposed to moisture before the lay-up period, and water ingress affected the battery content after the lifejacket was put back in storage.

โข Store such equipment in a secure, dry and cool place away from flammable materials;
โข Keep batteries of any kind ๐ฎ๐๐ฎ๐ ๐ณ๐ฟ๐ผ๐บ ๐บ๐ฒ๐๐ฎ๐น ๐ผ๐ฏ๐ท๐ฒ๐ฐ๐๐ to avoid short circuit between the terminals;
โข For Lithium batteries in your work place:
ฬถ Assess where Lithium batteries are present and check, if possible, the condition of the battery and cover;
ฬถ Verify the expiration date of the battery;
ฬถ Regularly inspect batteries of lifejacket lights and other devices like radios as per planned maintenance schedule;
ฬถ If batteries are expired or damaged, remove them and dispose of them properly

You need to gather all the liferafts and have them ready to be landed at the port for shore service.
I know this is the most challenging part of the job since sometimes you need other crew and lots of muscles to move the liferaft.
Before we continue,
remember that liferafts are serviced once every 12 months by authorized servicing personnel or a company (which may be extended to 17 months).
Moving forward.
After landing the liferafts, newly serviced liferafts are provided right away, and certificates for each liferaft are issued.
Make sure to file these certificates in their corresponding certificate folder.
๐ฅHere's an interesting part.
There are specific liferafts that can be accepted for extended service intervals not exceeding 30 months in accordance with SOLAS III/20.8.3 and MSC.1/Circ.1328.
In simple terms, if you have these kinds of liferafts, you don't need to land your liferaft for shore service; instead, you have to perform a simpler task called an Onboard Annual Inspection.
To do this, a special tool is provided.
๐๐'๐ ๐ฎ๐ฐ๐๐๐ฎ๐น๐น๐ ๐พ๐๐ถ๐๐ฒ ๐ฒ๐ฎ๐๐.
You just have to plug the tool into the liferaft, wait for a few seconds, and you are done.
As long as the equipment is green or says pass, your liferafts are still in good condition.
Just don't forget to record or fill out the necessary paperwork.
There you have it. That's generally what you have to do with your liferaft's annual service or inspection.

Like seriously, who reads this manual onboard anyway? Why should 3rd mate spend time on it?
All kidding aside, there could be a PSC deficiency due to an outdated and inaccurate manual.
Do this to overcome this problem.
1. If new crew are onboard, let them read and sign the manual (usually within 2 weeks).
2. Check and update the instructions and procedures of the equipment that are actually being used onboard.
For instance, if the EPIRB is replaced with another brand, you need to replace the instructions or procedures on how to use that particular EPIRB in the SOLAS Training Manual.
The bottom line is that whatever equipment is actually used onboard must be reflected on the SOLAS Training Manual.

Main inspection items on the Fire Doors (Hinged Self-Closing)

When a fire alarm is raised by the fire detection system, all self-closing fire doors should automatically close.
โ
For manual release, push the release button at the back of the door. The door should be detached from the electromagnet. (See photos below.)

When the door is released, it should swing and shut tight. Check for complete seal.
โ
Fire door should be free from hold back arrangement or wedges.
โ
Check that the closing and latching mechanisms are working properly.
โ
Check the condition of structural parts such as frames, door faces, and edges.

Are lifeboats considered to be an enclosed space?
To answer this question, we first have to define what is an Enclosed Space.
An Enclosed Space as per IMO is defined as a space that has any of the ff:
1. limited openings for entry and exit,
2. inadequate ventilation,
3. and is not designed for continuous worker occupancy.
โTotally Enclosed Lifeboats obviously meets the 1st criteria in which it has limited openings.
โThe 2nd criteria, regarding inadequate ventilation, is also met.
Hereโs why:
Lifeboats are designed to be sealed from the environment to provide protection against harsh weather and sea conditions, which inherently limits natural ventilation.
Third Officers should properly ventilate the lifeboat before and during any maintenance or inspection activities to prevent buildup of toxic gas.
โThe 3rd criteria, is also applicable since lifeboats are intended for emergency use and are not meant to be occupied for extended periods.
Their primary function is to provide safe evacuation and temporary shelter in the event of an emergency, rather than serve as a space where personnel are expected to work or reside continuously.
With these, we can say a Lifeboat is considered to be an enclosed space.
๐In some companyโs Safety Management System (SMS), lifeboats may not be directly mentioned as enclosed space
however,โฆ
given that lifeboats meet the criteria, it is prudent to apply the same safety measures that are used for other enclosed spaces on the vessel.

In most ports, agent will usually board the vessel for clearance and they will require some documents especially if there is a crew change.
List of documents is sent by the agent well in advance so prepare these documents whenever possible,

Check lashing condition, not too tight or not too loose

Visually inspect the hull for deformations, defects or cracks especially near joining edge of fittings.

Check the drain valve, cap/plug for any dirt deposits

Check opening/closing condition of compartments for watertight integrity.

Visually inspect overall paint condition
โ
On/off test search light, canopy light and ships illumination light.
โ
Together with enigineers, test the engine ahead and astern for not less than 3 minutes.

Check the pressure gauge of accumulator of Rescue Boat davit if charged.

It basically means the engine has been prepared and is ready for maneuvering.

During departure, as an officer, you will need to notify the engineers of the departure time so they can prepare promptly.
Once finished with preparations, they will inform you that the engine is on standby.

During arrivals, SBE essentially means the engines are ready to be used for slowing down the vessel.
