Para sa mga Newly Licensed Professional Teachers 👨🏻🏫👩🏻🏫
MAGANDANG ARAW, LPTs.
Dahil marami na rin sa inyo ang nagtatanong, ibabahagi ko na lang dito sa post na ito. Narito ang mga hakbang kung paano ko nakuha ang aking PRC - Professional Identification Card (PIC).
1. Open your LERIS Account. Check mo sa Existing Transaction mo kapag naging ATTENDED na yung dating PENDING sa Face-To-Face Mass Oath taking mo, then pwede ka na magpa-appoint for Initial Registration.
2. Click mo lang ang Initial Registration. Supply what is needed like a profession then Application Number na nasa NOA. Then submit. Makakapili ka na kung saang PRC Branch mo gusto.
3. Kapag okay ka na sa napili mong schedule, proceed ka na sa Payment. ₱1,050.00 po ang mababayaran.
Here's the breakdown:
•Registration Fee - ₱600.00
•Annual Fee - ₱450.00
•Transaction fee - ₱8.00 kapag Gcash ang Mode of Payment mo.
4. If successful na, download mo ang updated Oath Form mo. Makikita din yan sa Baba ng Initial Registration transaction mo.
5. Now, it's time for you to prepared docs/requirements, listed below:
• NOA
• Printed Oath Form (Updated)
• Photocopy of Updated Cedula
• 3 pcs. Passport size picture with name tag (white background)
• Photocopy of Valid ID (Back to Back)
• Documentary Stamp (2 pcs. If Violet) (4pcs. If Brown)
• Picture mo nung oath taking ceremony (for proof)
6. After mo makumpleto ang mga requirements, at schedule mo na pumunta ka na sa kung saang PRC Branch ka nagpa-appoint. Better na mas maaga ka to secure a priority number. Lapit lang kayo sa staff na makikita niyo sa labas.
7. Kapag naipasa mo na ang docs, wait for your name to be called na lang. So far, narerelease nila kaagad ang PRC ID.
As instructed by the staff to me, Registration Number seen in your PRC ID serves as your License Number while yung nasa likod na Number nakasulat pa-side will be the Serial Number.
Note: Maari nyo na ring isabay sa pagpapa schedule nyo ng Initial Registration ang pagkuha sa inyong mga CERTIFICATES at AUTHENTICATIONS (by schedule of appointment din po ito at each certificates and authentications ay 75pesos)

*Certificate/Authenticate of Good Standing
Reminders:
Hindi po kayo bibigyan ng PRC ng PIC (ID) kung hindi kayo nakapag OATH. OATH IS A MUST!
