Boys, it’s going so fast, isn’t it, do you feel it too?

Giving you all the tools to instil the skills we want you to develop, that part is effortless.

Nurturing you and opening your eyes to the world around you and the wider beyond you haven’t seen yet, that’s also effortless.

Seeing you take from us and every experience that surrounds you and watching you use that to grow into the people you’re going to be, that’s exciting.

A few thoughts as a ST3 equivalent Emergency Medicine registrar in one of London's Major Trauma Centre. Just to get one thing clear, I love what I do & every day I’m grateful for the opportunity to be an EM doctor - I moved mountains to get here and I don’t want to leave. Striking for change and pay restoration this week, here’s why:

Apparently a there's not much that’s 'junior' in what I do:

Although under supervision at this grade, I mostly work autonomously assessing patients, admitting them into the hospital to other specialties for further treatment or safely discharging them home with an outpatient plan. Appreciate the senior advice on difficult cases when needed which is less frequent these days. I also teach and advise SHOs on their cases helping them to get to where I am and beyond.

Working 10hr days with 2x30min breaks (the 2nd one rarely happens) and 11hrs at night. In the ED full-time, I'm averaging 48hrs per week - some weeks more (about to start a 6-day week including Sat & Sun - 60hrs) and some less (40hr week in 4 days - isn’t that a full working week for most people?). I work 1 in 3 weekends and 8 night shifts every 9 weeks.

I'm the go-to registrar if a trauma call comes in. I’ll be the one in the team, the first one you’ll see if God forbid it’s you coming in, doing the primary survey of a trauma patient which is the very first assessment picking up life threatening airway, breathing, cardiovascular, neuro or limb/vision issues and treat these immediately.

During the night shift, the lead registrar & I take charge of the dept. With 3 x SHOs, we see every patient coming in overnight. I'll be in charge of the resuscitation room seeing the blue-light ambulances with acutely unwell patients and manage any front of house issues with the triage nurses. The ST4+ lead reg will have overall responsibility for the dept ensuring the wait time is kept as low as it can be and is on hand for acutely unwell mental health patients who may need emergency measures. We both field calls, sign off every ECG and blood gas as somehow over the last few years these have become registrar-only jobs. Together the team, including nurses, EDAs & admin staff, keep this gateway to the rest of the hosp running smoothly until daylight.

Days are filled with nerve blocks on broken hips, arterial lines, chest drains & bedside ultrasound scans. Love that I have these skills to help someone that needs them. The hours spent mastering them and new ones are well worth it.

Passionate abt I teach and mentor medical students - the doctors of tomorrow. Increasingly hoping the sacrifices they’re making will be worth it.

Striking as part of industrial action this week because the conditions I work in are worsening, patient wait times are just getting ever longer leading to worsening care, and despite my growing skills and experience, my pay stagnates and reduces in real terms every year.

Buried my head in the sand for years not wanting to calculate my rate because I knew that once I did I wouldn't be able to unsee that number. But I just worked out that my hourly rate is £17-21/hr after tax & student loan…to be your on one of the worst days of your life🫡

Tomorrow is the 1st Wed of Aug when we get an influx of fresh new doctors and most other doctors are changing over to new their new rotations/jobs.

Thought I’d jump in ahead and add one to the grid for a quiet update of my own. Your girl started a new job exactly one month ago at Imperial College Healthcare NHS Trust 🙌🏽

Having never been at a major trauma centre before, probably the scariest thing for me for being called to a trauma call. 7 in one day in the first week ripped off that bandaid pretty quick đź« 

Good luck to all the new FY1s starting tomorrow. That imposter syndrome may kick in but in that moment just remember you’re exactly where you worked so hard to be. You got this 💫
