STEP UP WAR AGAINST KALA-AZAR AND OTHER NEGLECTED DISEASES

World Health Organization (WHO) classifies 20 diseases as neglected, and Visceral Leishmaniasis, famously known as kala-azar, is among them. These diseases are often overlooked by decision-making honchos, contributing to their neglect.

Kala-azar is a potentially fatal disease with the propensity of thriving under poverty conditions in penurious populations living in remote, rural areas and urban slums, exacerbating the poor into a vicious cycle of poverty and disease.

This has been cemented by Filip Meheus et al. (2013), who conducted research in North Sudan to know the economic burden of Leishmaniasis, and appallingly, the study indicated an estimated median cost at USD 450 with over 75% of the households reporting catastrophic out-of-pocket expenditures.

Following transmission through bites of infected sandflies, clinically, it presents with fever, anaemia, hepatosplenomegaly (enlarged liver and spleen), lassitude and wasting and nearly always results in death if left untreated in over 95% of cases. Globally, an estimated population of 350 million in about 88 countries are in jeopardy and pregnable to the Kala-azar menace.

However, in 2017, 94% of new cases reported to WHO occurred in seven countries: Brazil, India, Somalia, Kenya, South Sudan and Sudan. Kenya is sedulously reporting cases where transmission foci remain in Baringo, Isiolo, Marsabit, Pokot, Turkana and Wajir counties.

In 2018, the status of endemicity of kala-azar released by WHO indicated that Kenya recorded between 500-999 new kala-azar cases. The 2nd Kenya National Strategic Plan for Control of Neglected Tropical Diseases (NTDs) 2016-2020 has sagaciously captured disease-specific, cross-cutting, ambitious, impact-oriented targets and strategies that need to be implemented to control Kala-azar morbidity across the affected counties. However, some strategies have become fallacious and murky due to meagre resource allocation and implementation slough.

In the spirit of universal health coverage, the national and County governments and partners must collaborate and reinvigorate the laid-down strategies to eliminate Kala-azar.

As a country, we cannot afford to give a wide berth and turn a blind eye to this minacious disease impoverishing the penumbra communities. Article 43 (1a) of the current Kenyan constitution is unequivocally pellucid; it envisages that every person has the right to the highest attainable standard of health, which includes the right to health care services, including reproductive health care.

Therefore, we want to see the exertion and attention accorded to the COVID-19 pandemic replicated for Kala-azar disease. The earnest effort that Kenya put forth to be recognized by WHO at the zenith of global recognition as having attained certification of guinea worm disease following the recommendation of the International Commission for the Certification of the Dracunculiasis Eradication (ICCDE) at its 12th meeting in Geneva from 15-16 February 2018 can be emulated for Kala-azar elimination.

There is a truth in the adage that prevention is better and cheaper than cure; as Kenya, we are at the vantage point if we cobble up and galvanize our ingenuity toward formulating a kala-azar elimination plan.

Entomologically, vector control mechanisms will be the major fulcrum and panacea in the elimination process; this will involve attacking the sandfly by using myriad techniques with an aim to reduce vector density leading to reduced morbidity and mortality.

One of the ab initio techniques is utilizing geospatial mapping to earmark the hotspots. Thereafter, collate and analyse sandflies' bionomics in both peri-domestic and sylvatic vegetation niches since they have perilous implications for the effectiveness of vector control activities.

As a collateral benefit, indoor residual spraying (IRS) using modern hand compression pumps, provision of insecticide-treated nets, and environmental modification, which comprises of changes to the home or surrounding environment, will go along in ameliorating the Kala-azar conundrum.

Through espousing technology and innovation, biological alternatives to IRS can be explored to control sandflies by suppressing sandfly populations or adversely manipulating and abating their pathogen transmission competence.

Genetic approaches such as CRISPR-Cas, a nifty avant-garde molecular technique in the research and development market, could be potentially applied to generate and release sterile male sandflies to stymie vector reproduction.

Other methods that can be employed include the ‘lure-and-kill’ method, which involves using a synthetic copy of a male pheromone to attract female sand flies towards insecticide-treated areas.

In the spirit of One Health, all stakeholders from Human, Livestock and Plant health including cross-border counties and countries, should amass resources to revitalize the supply chain maturity continuum, which includes commodity security, blood and blood products, robust and sophisticated equipment for diagnosis and treatment, capacity building of staff and community. Achievement of these ambitions will be in tandem with Accra Declaration (June 2012) and World Health Assembly (WHA, 2013) resolution 66.12.

In some African farming communities, when a horse is sick to the point of death, it takes a woman, who has known only one man in her entire life, to jump over it and the horse will be healed. While, some school of thought says it must be a virgin woman that can achieve this; the most popular thread has it that it must be a faithful wife, who has only ‘known’ her husband alone, all through her life.

This practice can be said to be one that extols decency, morality and virtue. After the woman jumps over the horse, some few minutes later, the horse is expected to rise and start walking again. If it were the other way round and the woman lies, if she jumps over the horse, it will die instead of being healed.

As seen in the video below, the woman, who was considered ‘pure’, was asked to jump over the sick horse and few minutes later, the horse ran away with huge steps and the young men around rushed to pick up the woman and sing her praises.
