Monitoring of Oxygen saturation would be useful in patients who have respiratory symptoms and not in regular people. The pathogenosis of how Covid invades is still a research topic. The present thoughts are that it sits on a receptor called ACE2 which is present in all parts of our bodies but more in the lungs ( due to the surface area??) and destroys the underlying tissue creating a bad reaction (inflammation). The body reacts to it and thereby causes more inflammation and small clots in the blood vessels. These block the vital organs causing death. Therefore there is multi organ failure in the very sick patients.
Now how your body reacts to the virus is primarily determined by your immunity status, genetic factors and presence of other diseases or disorders. Again we see that 80-85% people are asymptomatic or very few symptoms because of above reasons.
So, the question of whether pulse oximetry would detect respiratory failure specifically in Covid infection is debatable. Pulse oximetry will detect that your blood is not getting enough oxygen in any setting whether asthma, pneumonia, shock cardiac arrest, etc and it is not sensitive enough.
Would like to end it by saying that any person who is having a pulse oximetry reading of less than 93% has to be in hospital to be observed and treated.
