HS can be classified into three stages using the Hurley staging system, which has been in use since 1989.

• Stage 1 (Mild): Single isolated lesions, no sinus tracts or scars.
• Stage 2 (Moderate): Single or multiple lesions with sinus tracts.
• Stage 3 (Severe): Widespread areas with sinus tracts and scarring.

Most patients are in Stage 1, with Stage 2 affecting about one-third, and only 4% reaching Stage 3.
