This is our release form for photo submissions to be used for publishing. Please be sure to copy and paste all info, and send it in your email with your submissions.

I will not accept any submissions without this release. Please be sure to initial all areas so that we know you have read the full agreement. Thank you. Contracted this ______ day of _____________ 20_____ between the individual signing below, hereinafter referred to as the PROVIDER, and Perfectly Pinup Magazine hereinafter referred to as the magazine. This release covers photos, artwork, articles, poetry, and any other content provided for the magazine. In consideration of my engagement as a content provider and for good and valuable consideration herein acknowledged as received, I hereby grant the magazine, legal representatives and assigns, those for whom the magazine is acting, and those acting with her authority and permission, the license to use in her name or otherwise, and re-use, publish, republish photographs, artwork, articles and content of me or in which I may be included as well as articles I have produced, in whole or in part, or composite in conjunction with my own or a fictitious name, or reproduction thereof in color or black and white, or otherwise made through any and all media now or hereafter known, for illustration, promotion, art, or any other purpose whatsoever related to Perfectly Pinup Magazine and any other promotional tools. I also consent to the use of any text used in conjunction therewith. I understand there is no commission for provided materials unless otherwise stated. Perfectly Pinup Magazine cannot, and will not provide free magazines or materials to me. x________________

I hereby agree to release, discharge and agree to save harmless the magazine, legal representatives and assigns, and all persons acting under her permission or authority, or those for whom she is acting, from any liability by virtue of blurring, distortion, alteration, optical illusion, electronic manipulation, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said photos, artwork or content or in any subsequent processing thereof, including without limitation any claim’s for libel or invasion of privacy. Content will only be altered in relation to the magazine and will only be done in the best interest of the provider and magazine. All articles are subject to proof reading and may be edited for grammar in the best interest of the writer without jeopardizing the integrity of the article. x________________

I hereby warrant that I am of legal age (to be photographs) in the jurisdiction in which this release is executed and have the right to contract in my own name, that I have read the above authorization, release and agreement, prior to its ex*****on and am fully familiar with the contents thereof. This release shall be binding upon me, my heirs, my legal representatives, and assigns. I warrant that any IDs or other documents I have shown the photographer to establish my age are legal, valid, and not counterfeit or fraudulent. This release grants the magazine permission to publish and republish photos, articles, and content. x________________

I agree that all my statements are true and to the best of knowledge. x________________

I understand that submission of content is not a guarantee of publication. Content will be used as it is deemed fit based on timing, deadlines, themes, etc. x________________

I will notify all involved parties of the content that I have submitted for publication. By signing the agreement I agree that I have permission from all parties to submit the photos to this publication and it is my responsibility to notify them and I hold no fault to the publication for any reason. x________________
