The hypothalamus releases ADH. ADH acts on the kidneys to increase water reabsorption from the renal tubules, thereby increasing volume and hence, blood pressure. ADH also constricts blood vessels, thereby increasing peripheral resistance. Hence its other name - 𝐯𝐚𝐬𝐨𝐩𝐫𝐞𝐬𝐬𝐢𝐧!⁣
⁣
𝐇𝐨𝐰 𝐝𝐨𝐞𝐬 𝐭𝐡𝐞 𝐛𝐫𝐚𝐢𝐧 𝐤𝐧𝐨𝐰 𝐰𝐡𝐞𝐧 𝐭𝐨 𝐫𝐞𝐥𝐞𝐚𝐬𝐞 𝐀𝐃𝐇?⁣
The brain stem and hypothalamic centers are notified of the need for a change in BP in several ways:⁣
Venous, cardiac, and arterial 𝐛𝐚𝐫𝐨𝐫𝐞𝐜𝐞𝐩𝐭𝐨𝐫𝐬 (carotid and aortic) respond to drop in BP by reducing their normal rate of firing to the BP centers in the brain stem and hypothalamus. Normally, baroreceptor firing INHIBITS the sympathetic output, (but stimulates the parasympathetic system), and INHIBITS the hypothalamus from secreting ADH.⁣
⁣
Decreased BP, then reduces baroreceptor firing. Decrease baroreceptor firing increases sympathetic outflow (and decreases parasympathetic outflow), with a consequent rise in BP. Decreased BP also acts via decreased baroreceptor firing to increase ADH secretion by the hypothalamus, which in turn also contributes to a rise in BP by increasing vasoconstriction and fluid retention.⁣

𝐎𝐬𝐦𝐨𝐥𝐚𝐥𝐢𝐭𝐲:⁣
Increased serum osmolality (mainly due to an increase in sodium) acts in ADH secretion. (𝐎𝐬𝐦𝐨𝐥𝐚𝐥𝐢𝐭𝐲 is a property of a solution that increases with the number of dissolved particles in the solution). It’s as if the body through evolution came to realize that increased serum osmolality commonly means dehydration and the need to restore 🩸 volume and pressure. So the hypothalamus the secretes more ADH. The hypothalamus also responds to serum hyper osmolality by inducing thirst to increase fluid input.⁣

𝗪𝐡𝐚𝐭’𝐬 𝐒𝐨 𝐈𝐦𝐩𝐨𝐫𝐭𝐚𝐧𝐭 𝐀𝐛𝐨𝐮𝐭 B𝐏?⁣⁣⁣⁣
Without BP, you die quickly, as pressure is necessary to drive 🩸 flow (perfusion) through the tissues. With BP that is too high, you may die acutely: a 🩸 vessel may burst suddenly, causing a stroke (loss of circulation to the brain); or there may be longer-range problems, such arteriosclerosis. The body has developed multiple mechanisms to ensure the proper control of 🩸 flow.⁣⁣⁣⁣
🫀⁣⁣⁣⁣
𝗪𝐡𝐚𝐭 𝐂𝐨𝐧𝐭𝐫𝐨𝐥𝐬 𝐭𝐡𝐞 🩸 𝐏𝐫𝐞𝐬𝐬𝐮𝐫𝐞? See 👆⁣⁣⁣⁣
The key factors that regulate systemic BP are 𝐩𝐞𝐫𝐢𝐩𝐡𝐞𝐫𝐚𝐥 𝐫𝐞𝐬𝐢𝐬𝐭𝐚𝐧𝐜𝐞 - particularly as mediated through 🩸 vessel constriction and 𝐜𝐚𝐫𝐝𝐢𝐚𝐜 𝐨𝐮𝐭𝐩𝐮𝐭 (heart stroke volume x heart rate, normally 5-6 liters/min). An increase in any of these will increase BP.⁣⁣⁣⁣
⁣⁣⁣⁣
Increased 𝐩𝐞𝐫𝐢𝐩𝐡𝐞𝐫𝐚𝐥 𝐫𝐞𝐬𝐢𝐬𝐭𝐚𝐧𝐜𝐞 raises pressure by impeding 🩸 flow, resulting in backup of 🩸 in the arteries, where BP is measured. Resistance increases mainly through vasoconstriction, particularly at the arteriolar level, but may also increase somewhat through elevated friction in cases of increased viscosity.⁣⁣⁣⁣
⁣⁣⁣⁣
𝐂𝐚𝐫𝐝𝐢𝐚𝐜 𝐨𝐮𝐭𝐩𝐮𝐭 is extremely important in determining BP, for if the heart were to suddenly stop, with no change in overall blood volume or peripheral resistance , the blood pressure would drop to about 5-10 mm Hg. ⁣
⁣⁣⁣⁣
Increasing the 𝐛𝐥𝐨𝐨𝐝 𝐯𝐨𝐥𝐮𝐦𝐞 increases pressure by increasing cardiac output. 🩸 volume is partly a function of balance of fluid intake and output. Output occurs mainly through the kidneys. 🩸 volume also depends significantly on osmotic and other forces that tend to keep fluid within the blood vessels, as opposed to extravascular spaces.⁣⁣⁣⁣

Because gonorrhea is usually diagnosed with molecular tests that do not provide antibiotic susceptibilities, we are often unaware the patient may have drug resistance. When resistance is suspected, standard culture with antimicrobial susceptibility testing may need to be performed to devise an effective treatment regimen!⁣

𝐍𝐢𝐩𝐚𝐡 𝐚𝐧𝐝 𝐇𝐞𝐧𝐝𝐫𝐚 are related paramyxoviruses that cause severe central nervous system infections that emerged in recent years.⁣

🦇𝐌𝐞𝐭𝐡𝐨𝐝 𝐨𝐟 𝐭𝐫𝐚𝐧𝐬𝐦𝐢𝐬𝐬𝐢𝐨𝐧⁣
Two bats are flying next to each other; one is peeing on a 𝐇orse named 𝐇endra and the other is peeing on some dates that have fallen off a date palm tree. Hendra is spread to humans via urine of infected horses, while Nipah is spread directly to humans via bat urine, often on date palm sap. Nipah is spread to people by direct contact with animals such as bats or pigs, or their body fluids. The classic transmission though is by bats urinating in receptacles designed to collect date palm sap. Humans are exposed to the Nipah virus when they consume the sap. Infection CAN also be spread person to person via human body fluids when it comes to Nipah but to date no human-to-human transmission has been found with Hendra.⁣

Symptoms range from mild influenza-like illness to fatal respiratory or neurologic disease. The Nipah virus has led to major outbreaks in and . Of the two, Nipah is more dangerous because it causes more severe disease and at times is able to spread from human-to-human.⁣

𝐍𝐚𝐬𝐭𝐲 𝐍𝐲𝐬𝐭𝐚𝐭𝐢𝐧 cruises down the esophagus 👆 killing fungi on the walls of the esophagus and stomach. In one end 👄and out the other!!!💩⁣⁣
⁣⁣
🍄𝐌𝐞𝐭𝐡𝐨𝐝 𝐨𝐟 𝐚𝐜𝐭𝐢𝐨𝐧:⁣⁣
Nystatin, like amphotericin B (see previous post👉) binds to ergosterol, increasing the permeability of the cell membrane and causing lysis.⁣⁣

🍄𝐃𝐫𝐮𝐠 𝐝𝐞𝐥𝐢𝐯𝐞𝐫𝐲:⁣⁣
Think “𝘕𝘢𝘴𝘵𝘺 𝘕𝘺𝘴𝘵𝘢𝘵𝘪𝘯” with that guns a’ blazing’ face moving down the esophagus to remember that this drug is too toxic to take parenterally (IV). It is only used on mucous membranes and topically on the skin. ⁣⁣

🍄 𝗪𝐡𝐞𝐧 𝐭𝐨 𝐮𝐬𝐞? ⁣⁣
Since it is not absorbed into the GI tract, oral nystatin can be used to treat oral and esophageal infections from yeast and fungi. You will order nystatin on the wards as 𝐍𝐲𝐬𝐭𝐚𝐭𝐢𝐧, 𝐒𝐰𝐢𝐬𝐡 𝐚𝐧𝐝 𝐒𝐰𝐚𝐥𝐥𝐨𝐰 for treatment of oral, esophageal, and gastric candidiasis. It is also given topically for vaginal candidiasis. ⁣

The classic anti fungal med is amphotericin B. Most species of fungi are susceptible to it, and although it has many side effects (therefore referred to on the wards as ampho𝐭𝐞𝐫𝐫𝐢𝐛𝐥𝐞), it remains the first line option for many serious systemic fungal infections:⁣⁣
⁣⁣
✅Systemic 𝘊𝘢𝘯𝘥𝘪𝘥𝘢 infections⁣⁣
✅Cryptococcal meningitis; combo flucytosine⁣⁣
✅Invasive Aspergillosis⁣⁣
✅Invasive Sporotrichosis⁣⁣
✅Mucormycosis⁣⁣
✅Severe pneumonia and extra pulmonary Blastomycosis, Histoplasmosis,, and Coccidiomycosis⁣⁣

1️⃣IV delivery⁣⁣
2️⃣Fungicidal by binding to ergosterol in fungal cell membrane, causing membrane disruption and osmotic lysis of cell⁣⁣
3️⃣Nephrotoxicity⁣⁣
⁣⁣
𝐀𝐦𝐩𝐡𝐨 𝐁 𝐝𝐞𝐥𝐢𝐯𝐞𝐫𝐲 𝐩𝐫𝐨𝐜𝐞𝐬𝐬⁣⁣
These side effects are VERY common so it usually given with aspirin or acetaminophen to prevent febrile reactions. Daily BUN and creatinine levels are drawn to monitor kidney function. To speed travel through the kidneys and decrease renal toxicity hydrate with normal saline. Electrolyte replacement due to urinary excretion of potassium, magnesium, and bicarbonate.⁣⁣

Newer preparations add different lipids to decrease nephrotoxicity, making it less 𝐀𝐦𝐩𝐡𝐨𝐭𝐞𝐫𝐫𝐢𝐛𝐥𝐞!🐸⁣⁣
