Consecutive case using laminar bone sheets
Poor patient with Severe bone loss and peri implantitis on #9 implant. History of heavy smoking. Implant was easily removed as well as cystic lesion expanding to palatal bone. A large fenestration on buccal bone however absolutely no palatal plate detected. Essentially a 2 walled defect with decent interseptal bone.
With such large defects, I like to utilize a demineralized cortical bone sheet for:
👉structural support to reduce pressure on graft.
👉cortical bone sheet can be trimmed to adapt around defect.
🔥Superficial cuts into the bone sheet to bend into the defects-Relieving the stiff memory for better adaption.
👉The bone sheet was used to rebuild the entire palatal wall for initial graft containment.
👉Handles like d-ptfe membrane. Doesn’t stick like collagen membranes.
👉Here I stabilized bone sheet with monofilament suture That can also be done with screws or pins.
PRF placed for early angiogenesis and wound healing. Slow resorbing and excellent space maintenance for 6 months or more.

Heavy Calculus Removal!!
This is an extreme case of calculus tooth extraction. In dentistry, calculus or tartar is a form of hardened dental plaque. It is caused by precipitation of minerals from saliva and gingival crevicular fluid in plaque on the teeth. This process of precipitation kills the bacterial cells within dental plaque, but the rough and hardened surface that is formed provides an ideal surface for further plaque formation. This leads to calculus buildup, which compromises the health of the gingiva (gums). Calculus can form both along the gumline, where it is referred to as supragingival, and within the narrow sulcus that exists between the teeth and the gingiva, where it is referred to as subgingival.
Calculus formation is associated with a number of clinical manifestations, including bad breath, receding gums and chronically inflamed gingiva. Brushing and flossing can remove plaque from which calculus forms; however, once formed, it is too hard to be removed with a toothbrush. Calculus buildup can be removed with ultrasonic tools or dental hand instruments.

This case was of a child who came to the Health Unit reporting that she felt pain when she ate and decided to get a screw to move, that's when she felt pain and involuntarily bit.
An X-ray was performed, there was no permanent compromise and the extraction was performed. Remembering that in this case the tetanus vaccine was checked.

An apicectomy may be indicated in situations where there is a failure of conventional endodontics such as a sclerosed root canal, inaccessible lateral canals, apical root fracture, a fractured root canal instrument that cannot otherwise be removed, a symptomatic apically extruded root filling, an open apex that cannot be sealed using a conventional root filling, an apical cyst needing to be removed, or when an apical biopsy is indicated.

Entrapped floss retrieval. Floss left subgingivally lead to periodontal abscess and soft tissue fenestration. This is due to failure of another colleague to retrieve the floss which was used for retraction during old composite veneers fabrication. #laser removal of a traumatic fibroma on the lip of a gentleman who kept on accidentally biting it until it grew to a pedunculated mass. A drop of anesthetic around it and it was done within a few minutes. No bleeding, no pain. #waterlase #biolase #biologicaldentistry Via @sarkissiandds
When a patient is hard to numb, rather than overdose them with anesthetic, the #stabident is a great alternative and not so hard to do. A quick look at the X-ray and if the bone and root anatomy permit, proceed. Anesthetize the buccal aspect first with 1/2 carpule of articaine. I use an electric blue 1:1 handpiece with high torque and RPM around 3000. Hold the pin firmly and perpendicular to the bone. Best to go through attached gingiva. Otherwise you have to spend some time to find the osseous access hole with the needle under the movable gingiva (which obviously moves). Inject 1/3 Carpule intraosseously very slow and preferably use a Non-epinephrine anesthetic. Via @sarkissiandds #biologicaldentistry #oralanesthesia
Left TMJ arthrocentesis This patient has acute symptoms (3wks duration) of limited range of motion and pain during function of the left TMJ. We planned arthrocentesis to reduce inflammation of the left joint. A Y-shaped double cannula is placed into the superior joint space. Saline is then flushed through the joint space. Once in the superior joint space, I hook the cannula up to an IV line and like to flush >100ml through the space. Steroid is then placed in the joint and the cannula removed. This is performed with IV sedation and the mandible is manipulated to help improve range of motion. This will not solve all TMJ problems, but is great at reducing inflammation and improving pain. Tip how to remove composite remnant before delivery of final restoration. ⠀ We used long term partially bonded provisional restorations. So after removal we were not able to get perfect surface for of final restoration due to composite remnant. Pasta Rossa by @anaxdent was used to identify access zones. Enjoy. ⠀ Via @maximbelograd
