CATARACT SURGICAL CARAVAN
Date of Screening: Feb 17, 2024, at Capitol Gym, Borongan City
Date of Operation: Feb 17-21, 2024, at ESPH (Eastern Samar Provincial Hospital), Borongan
Deadline for Linelist of patients: January 29, 2024, Monday (c/o RHU/HRH assigned)
Pls ensure contact details of the client (if none, kindly provide contact details of Punong Barangay & HRH assigned)
To submit the linelist to Ma'am Meghann through email [email protected] or messenger (template of linelist will be provided)
Selection of criteria:
โก With identified CATARACT or serious eye problems that need eyeglasses (20/100 and above base on Snellen chart or the Peek acuity application that can be downloaded through play store), then patients with hearing problems
Slots per LGU:
โ 40 per LGU (except for Guiuan & Borongan with 80 each)
SPECIAL INSTRUCTIONS:
1. Patients should be ready for surgery & bring their necessities for a possible overnight stay on February 17, 2024 (screening date).
2. Only 1 bantay per patient is allowed.
3. No free accommodation & meals - pls inform the client & family.
4. Social media announcement - c/o all RHUs
5. Other serious eye and hearing problems may be included.
6. No age limit for patients, preferably INDIGENT.
7. Patients screened by BKMM who were previously identified as operable for cataract surgery but did not avail should be prioritized.
8. Initial screening should be done at the RHU (before sending the final linelist).
โNo walk-ins allowed.
