A. Reservist Officer
General Qualifications for Commission in the PN Reserve Force:
1. The age of the applicant must not be less than twenty-one (21) years nor more than sixty-four (64) years of age at the time of Appointment as a reserve officer
2. Applicant must be a Filipino citizen
3. Every applicant must pass the physical examination standards conducted by an authorized AFP hospital or civilian/government hospitals and the physical examination report to be validated by the Major Service Chief Surgeons/The Surgeon General
4. Baccalaureate degree holder with at least four (4) years of study�Pre-entry Training graduate (Basic ROTC/ Basic Citizen Military Training/ Military Orientation Training for PN Affiliated Reserve Unit)
5. Cleared by Office of the Deputy Chief of Staff for Intelligence, J2
Documentary Requirements for Commission in the PN Reserve Force:
1. APPLICATION FORM DULY ACCOMPLISHED (Fill-up the information required, do not leave blank: write not applicable)
2. AFFIDAVIT OF NO PENDING CASE (Form to be filled up and duly notarized)
3. WHOLE BODY PICTURE (Postcard Size – Uniform of Current Position & or Business Attire)
4. PERSONAL DATA SHEET OR RESUME (With 2X2 Picture, White Background in Uniform of Current Position
5. NSO BIRTH CERTIFICATE (Original Copy)
6. COLLEGE DIPLOMA AND TRANSCRIPT OF RECORDS (Authenticated by School Registrar)
7. BOARD CERTIFICATE/BOARD RATING/PRC LICENSE or AFPSAT (Authenticated by issuing office)
8. SEA SERVICE RECORD/CERTIFICATE OF EMPLOYMENT
9. MEDICAL/DENTAL EXAMINATION REPORT(From Private Hospital should be validated by AFP Hospital)
10. COMMAND CLEARANCE

1. Requirements be submitted at O/NR2 or NRCens in One (1) original folder & Two (2) photocopies folders with tabbing (RED).

3. Submission of Medical/Dental exam report shall be already validated by AFP Hospital
4. Secure a directive from NRMC or NRCens prior validation to AFP Hospital
B. Reservist Enlisted Personnel
General Qualifications for Enlistment in the PN Reserve Force:
1. The age of the applicant must not be less than eighteen (18) years nor more than sixty at the time of Enlistment
2. Applicant must be a Filipino citizen
3. Pre-entry Training graduate (Basic ROTC/ Basic Citizen Military Training/ Military Orientation Training for PN Affiliated Reserve Unit
4. No criminal record
