سلام عليكم. الكثير من الزملاء بعت يسألنى عن خطوات الإعداد و تخطى إمتحان ال EACVI فى الايكو عشان كده جمعتلكم كل الخطوات و الإستفسارات و الأسباب اللى تخليك تنجح بأمر الله

✍️ البوست ده لأطباء القلب المهتمين بدراسة الايكو و تحديدا بامتحان البورد الأوروبى EACVI:

🛑 الجزء الأول: تعريف بالامتحان

✓ الامتحان بيتم من خلال الجمعية الأوروبية لموجات القلب EACVI
1. Adult TTE
2. TEO
✓ الامتحان بيقدملك خبرة عملية و معلوماتية كبيرة جداً و ده بيعتمد برده عليك إنك تقدر تستفيد منه بالشكل الأمثل
✓ لكن كمان الشهادة معترف بيها فى الجمعيات دى 👇
1. British Society of Echocardiography
1. Austrian Working Group of Echocardiography
Working group on echocardiography and imaging modalities in cardiology,
3. Croatian Cardiac Society

👈 و بناء ع المذكور فى الموقع إنك تقدر تقدم لعضوية BSE مثلاً و تقدم على وظائف لكن التفاصيل بالظبط مش متاحة عندى حالياً.
👈 صلاحية الشهادة: ٥ سنين بعد إجتياز الجزء النظرى و العملى.

✓ نظام و ميعاد الامتحان :
١. نظرى online :
*مرتين فى السنة اخر يونيو و اخر نوفمبر
*بيبقى عبارة عن 2 sessions
A) 50 MCQs, videos or still images, 90 minutes
> 10 minutes break
B) 75 MCQs, no cases or videos included, 110 minutes

👈 الوقت كويس جداً للامتحان و الاسئلة مش طويلة و مافيش رغى فيها يعنى السؤال سطرين تلاتة بالكتير.

* النجاح في النظرى لازم يبقى فى كل امتحان لوحده مش فى المجموع و ال passing score بتحدد كل مرة بناء على مستوى الامتحان و الدرجات و النتيجة خلال أو بعد ٨ أسابيع من خلال الميل.
* الجزء العملى عبارة عن Logbook بيتبعتلك بعد ما تنجح باذن الله مطلوب منك تجمع ٢٥٠ حالة ايكو و ٦ حالات ليهم بعض التفاصيل الخاصة على مدار ١٢ شهر سواء قبل أو بعد الامتحان المهم تبعت الحالات فى فترة لاتتعدى ١٢ شهر بعد الامتحان مع الأخذ فى الإعتبار أنهم بيردوا عليك بعد ٣ شهور و ممكن يطلبوا تعديلات.

✓ حجز الامتحان:
* التقديم بيفتح قبل ميعاد الامتحان ب ٣ شهور و كل ما تقدم بدرى بيكون أفضل لأنهم بيقبلوا عدد معين.
* بتملى استمارة من خلال لينك التقديم و بتكتب بيانات supervisor الايكو بتاعك و مافيش أى اشتراطات طلبوها بخصوص النقطة دى.
* تكلفة الامتحان ٦٦٠ يورو. أما إذا عملت silver membership و ده ب ٧٦ يورو بتقل تكلفة الامتحان ل ٤٣٢ يورو.
* الغاء الحجز قبل الامتحان ب ٤ اسابيع: بتسترد ٧٠٪ فقط
* الغاء الحجز بعد كده مفيش استرداد.
* لا قدر الله فى حال عدم اجتياز النظرى مصاريف الامتحان التالى بتبقى ١٣٦ يورو تقريبا ع ما أذكر.
* فى نقطة مهمة للدفع هى أن دفع الفيزا بره مصر ليه limit و بالتالى هتحتاج حد يدفعلك غالباً.
* بعد الدفع بتعمل حساب على Proctor U و ده نظام خاص بالامتحان الاونلاين و ليه browser خاص بيه بتحمله من الموقع اسمه guardian و تدخل عليه عشان تحدد الساعة بالظبط اللى هتمتحن فيها هتلاقيه شغلك بعدها timer بيفضل يعد لحد الامتحان بالايام و الساعات (التوقيت اللى بيظهرلك بيبقى على حسب البلد اللى بتسجل منها عشان ماتتلخبطش)

🛑 الجزء الثالث: المذاكرة

👈 و دى تجربتى الشخصية:
١. ابدأ بكورس الايكو بتاع ال ASE و ال EACVI و دى موجودة فى مكتبة المروة فى القصر العينى أو ممكن تلاقيها مع زمايلك. فيها شرح ممتاز لأغلب الموضوعات بشكل نظرى و عملى واضح و منظم .
٢. مع الفيديوهات ابدأ نظرى من كتاب EACVI handbook مش textbook لانه كبير اوى. كتاب صغير و سهل جدا تقرا منه و سهل جدا تراجعه قبل الامتحان ع طول. و فرق جدا معايا فى الامتحان.
٣. دلوقتي تقدر تبدأ تذاكر بجد و عن نفسى بما سألت عن مصدر موثوق منه اختلفت الآراء حتى موقع الامتحان نفسه كتب امثلة فقط زى 👇

👈 باختصار إنت محتاج تشوف كتاب مش كبير منظم فى عرضه تلحق تراجعه قبل الامتحان لأنك لسه هتقرا كتب MCQs و إما تضيف للكتاب اللى معاك أو تجمع ملاحظاتك فى ملزمة تانية. أهم حاجة تعمل ملخصات أو highlights لأى مصدر بتذاكر منه عشان هتحتاس قبل الامتحان.
حتى الفيديوهات اللى بتبقى مع كتب النظرى مهم جدا تجمع الحالات الصعبة و تراجعها قبل الامتحان

👈 ال physics: ليها مذكرة موجودة مع كل اللى امتحنوا إلى جانب الاسئلة اللى كتب ال MCQ محتاج تفهمها و تحفظها كويس جدا عشان التفاصيل مهمة لكن مش عايزك تشيل همها بزيادة. فى الأول و الاخر هى موضوع من ضمن مواضيع كتير جداً و الامتحان نفسه مكنش معقد أوى فيها.

٤. ال Qbank:

دى أهم الكتب اللى محتاج تقراها و تفهمها و تنقل أو تلخص المهم من الأسئلة و شرحها فيديوهات الحالات

1. Echo Board review 500 MCQs
2. Echocardiogrphy case based review
3. Echo review self assessment

٥. فى مصادر كتير تانية و إنت و شطارتك كل ما تقرا و تشوف حالات أكتر هيفيدك أكتر.
٦. أمثلة على ده:
a. Cardiology quizzes/Echo Board review
أمثلة كتيرة جداً تابعها و تابع تشخيصها
b. Dr. Sedafy Board Course
مفيد جدا فى المواضيع اللى ممكن تبقى نقطة ضعف عندك زى ال physics/TEE/hemodynamics
c. 123 TEE course برده موجود عند مكتبة المروة فى القصر العينى إذا عندك وقت اسمعه

✓ وقت المذاكرة ملوش مدة محددة على حسب ظروفك و وقتك لكن محتاج ع الأقل اسبوعين - تلاتة قبل الامتحان متفرغ فيهم للامتحان.

🛑الجزء الرابع: قبل الامتحان

لازم تبدأ تجهز أشيائك أو عدتك قبلها بشهر.
١. محتاج كمبيوتر ليه مواصفات موجودة ع الموقع من اللينك ده 👇

٢. الكمبيوتر لازم يكون شخصى و كل حاجة متسجلة عليه باسمك. مهم جدا الشاشة تبقى كبيرة عشان لما تشغل فيديو ماتحتاجش تقتح window لوحدها وده هيوفر معاك وقت.
٢. النت: لازم يكون مستقر و سريع فى download/upload . أفضل حاجة كابل أرضى و بعديها الوافاى الأرضى. و يفضل يكون معاك USB Vodafone احتياطى، أنا عن نفسى اشتغلت بيها طول الامتحان كانت ممتازة جداً أحسن من النت الأرضى.
٣. المكان: الامتحان بيبقى من خلال مراقب اونلاين تبع proctor u و بيكون متابعك صوت و صورة و عامل access للكمبيوتر عندك. عشان كده لازم تبقى فى غرفة فاضية مقفولةحتى الشباك يكون عليه ستارة. الغرفة يكون فيها مكتب و كرسى و الكمبيوتر و مروحتك أو تكيفك أى حاجة تانية هيطلب منك تشيلها أو تغطيها . عشان تقلل وقت المراقب قبل الامتحان و توفر ع نفسك. مش مسموح ليك وقت الامتحان غير بوجود مشروبات فقط.
٤. أول ما تسجل و تفتح حسابك ع Proctor U اعمل إختبار الجهاز و النت عندك عشان إذا فى حاجة تعدلها.

🛑 الجزء الخامس: المراجعة

١. إبدأ بمراجعة المصدر الرئيسى ليك و الملخصات بتاعتك
٢. محتاج تراجع ملخصات ال MCQs سريعا و الفيديوهات المهمة
٣. مذكرة الفيزيكس مهم جدا تراجعها و تراجع معادلاتها كويس جدا لكن برده ماتخليهاش تاخد وقت المراجعة منك. فى الأول و الاخر هى شابتر.
٤. لازم تقرا EACVI handbook قبل الامتحان بيومين مثلا و ركز كويس معاه عشان بيجى منه أسئلة.
٥. فى مواضيع لازم تقرأها سريعا قبل الامتحان مباشرة لأنك غالباً هتلاقيها فى الامتحان زى:
📌 Diastolic dysfunction +++++++
📌 hemodynamics and Equations ++++
📌 Valvular +++++++++ (take care of classification exact values)
📌 CMs ( HCM and others and patterns of speckle) +++++++
📌 RCM +++++
📌 Artifacts, physics +++++
📌 TEE (MV scallops) ++++++
📌 page 314 EACVI handbook (multiple valves)

👉 Catch of the day
24 y/o female, non-specific symptoms.

✓ Echo study seemed normal except for IAS Aneurysm (clip).

✓ By obtaining different views I managed to detect stretched PFO about 6 mm (images).

📌 The clue was presence of IAS Aneurysm, which is highly associated with PFO ( 50-75% association).

👉 Take home message: always pay attention to any abnormalities u see and look for the associated findings.

70 y/o female patient. Diagnosed recently with breast cancer. Received radiotherapy almost 2 years ago.
2D showed MV board like thickineng and calcification more confined to the proximal leaflet and sparing the distal part in conjunction with AVC which is typical for radiation induced valvulopathy.

📌 Radiation induced valvulopathy.
=============================
1. Pericadial calcification most common and early (few months)

2. Valvulopathy: later and typically affecting AO>MV. AS>MS>MR.

3. AML>PML. With sparing of distal leaflet.

Welcome to CardioIMAGE 2017 in Cairo, Egypt!

Cardiovascular imaging is the flagship of the Egyptian Society of Cardiology and the National’s largest cardiovascular imaging congress.

This year’s the working group of Echocardiography in collaboration of non invasive imaging group set the main themes: are How to image the heart by different modalities and imaging in coronary artery disease.

The aim of CardioIMAGE 2017 this year is to create an exciting forum, in which both clinicians and scientists can present and discuss the most up-to-date technical and clinical findings on the broader spectrum of echocardiography alongside with Computed tomography, Magnetic resonance imaging, nuclear imaging to reach the accurate diagnosis in fast way. An educational track through 4 workshops in the four techniques (ECHO, CT, CMR and nuclear) will run throughout the congress, featuring several specifically designed basic courses and sessions.

CardioIMAGE in Egypt-(CIE) is more than just education and science. It is also a fantastic opportunity to meet the greatest experts in cardiovascular imaging in the Egypt, to exchange ideas and projects with colleagues who share the same clinical or scientific interest coming from all over the country.
Four Awards from the Egyptian Society of Cardiology in the form of full sponsorship for registration, accommodation and flight ticket to the best case submitted in Echocardiography, MultiSlice CT, Nuclear Cardiology and CMR.

- Organise teaching workshops.
- Recommend the establishment of ESC task forces to the committee for practice guidelines and policy conferences to publish appropriate guidelines and recommendation and to set up study groups to discuss and publish issues of contemporary importance to the working group.
