Last month, our fab Occupational Therapists Chloe and Emily featured on Rad Chat - the award-winning, first therapeutic radiographer led oncology podcast, designed for cancer patients, healthcare professionals, academics and researchers.

They discussed occupational therapy as a career, how they support patients and myths they wish people knew, and we're very excited that it's now available to listen to at https://orlo.uk/tCkOh or where you normally get your podcasts.

If you like the sound of Occupational Therapy, we've got a great opportunity to join the team! Find out more here 👉 https://orlo.uk/bJEw7

This week alongside our podcast episode with Richard Baker, the UK Engineering Manager GenesisCare we have an equipment special.
The is an image of the modulator within the linear accelerator.
The accelerator is where the electrons use the microwaves to gain speed and energy. They use the waves much like a surfer would by using the energy and speed of the wave to accelerate in towards the shore. The electrons get injected in from the gun at 0.4 the speed of light and leave the other end at 0.99 the speed of light. Around the world 7 times in a second! It is mounted horizontally in a gantry for high energy single or dual energy machines with klystrons, and vertically for low energy machines with magnetrons. Only about 33% of electrons emitted to the accelerator waveguide are actually accelerated.
A circulator is used to prevent reflected waves from reaching the magnetron/klystron.

This week alongside our podcast episode with Richard Baker MIET the UK Engineering Manager GenesisCare we have an equipment special. These images show the Electron Gun, which is responsible for injecting electrons into the accelerating waveguide. It accomplishes this using a heated cathode which thermionically “boils-off” electrons. These electrons are then accelerated toward a grounded perforated anode and are focused into a pencil beam by a focusing coil.

They are injected in at 0.4 of the speed of light. Roughly around the world 3 times in a second!

This week alongside our podcast episode with Richard Baker MIET the UK Engineering Manager GenesisCare we have an equipment special.
Picture 1: The Magnetron. You have one of these in your microwave oven, except these ones are a bit more powerful. They can go up to around 5 - 7 million watts.
No microwaves come out towards the patient, but they are used to accelerate electrons which is the radiation we use to treat patients.
Picture 2; This is the Thyratron which is basically a big switch. In the toilet cistern analogy it is our flush for the electrical pulse from the Modulator.

This week alongside our podcast episode with Richard Baker, the UK Engineering Manager GenesisCare we have an equipment special.
The is an image of the modulator within the linear accelerator.

This is the Modulator which stores high voltage and power. It does this in the same way a toilet cistern stores water. It then flushes or pulses the power to the next part of the system.

Or a more technical version:

This is the Modulator which stores high Voltage and Power up to 19,000V at 10MW. It does this by filling up a bank of capacitors in the same way a toilet cistern fills up with water. The Power is then flushed (pulsed) to the next part of the system in a pulse that lasts for only 3millionths of a second.

**Trigger Warning**

Podiatry Week

1. Assessing and fitting insoles to best fit a patient’s foot especially those suffering from painful pressure points under the foot. Just like dentists customise dentures, podiatrists customise insoles.
2. We carry out foot mobilisation to improve movement in the feet and reduce restriction and pain.
3. Assessing the range of movement and detecting restrictions. Tiptoe Foot Care every joint matters especially when your feet are the foundations of the body.

Podiatry Week

Podiatrists can carry out neurological tests to detect early sensory loss. Sensory loss can be common in diabetics and patients undergoing radiotherapy or chemotherapy treatments.
This picture shows the monofilament tool we use to detect light touch sensation.

**Trigger Warning**

Podiatry Week

1. Hand foot syndrome from chemotherapy shows a sudden red colour change on the palms and soles.
2. Hand foot syndrome - the colour change on the palms and soles can take on a more purplish colour (rather than red) in brown and black skin tones.
3 and 4. A common nail toxicity side effect from chemotherapy is ulcerations developing under the nails and becoming infected.
5. Fingernails are also affected by chemotherapy. This is a pseudomonas infection but it is important to identify the right bacteria when treating.

**Trigger Warning**

Podiatry Week

1. Podiatrists check the skin integrity and nails. We are looking out for ulcerations, circulatory changes and potential sensation issues.
2. Chemotherapy can often lead to painful areas of hard skin and corns. We remove it gently using a scalpel and this minimises painful pressure areas.
3. Identifying nail deformities such as fungal nails (onychomycosis). This can be a long term side effect from cancer therapies like chemotherapy.

**Trigger Warning**

Podiatry Week

1: Afni is using kinesiology to test individual muscle strength in the arms. It can affect the patient’s swing cycle in gait so can cause balance issues and increased strain on certain joints in the lower limbs.
2: Testing muscle strength in the hip muscles using kinesiology and detecting triggers points that need to be released to allow improved pelvic movement.
3: here Afni is using her anatomy in motion training to reintroduce movement back into the patient’s gait cycle, rebalance their posture using a series of exercises and the wedges allow us to open up certain joints in the foot gently.
4: Afni working to reduce nail thickness and nail deformities caused by the side effects of chemotherapy.
