Poor chest compressions are better than no chest compressions!
With being said everyone should strive to attend a reputable CPR/AED class.
Here’s a quick run down as a refresher:
1. Scene safety, initial patient assessment, PPE.
2. Check for life threatening bleeding.
3. Check for responsiveness, breathing, pulse.
4. Call 911 (or have someone else do it). Remember group mentality. People will just stand around not knowing what to do. Step and demand someone specifically to call 911.
5. Lay patient supine (face up) on a flat hard surface.
6. Give 30 compressions. 2 hands with fingers interlocked, centered on chest, shoulders over Chest with arms locked. Depth of at least 2 inches (adult). Infants and children is a depth of about 1.5 inches. Compression rate is about 100-120 compressions per minute. Allow chest to return to normal position after each compression.
7. Give 2 rescue breaths if you are comfortable doing that but studies show that hands only cpr can be just as effective in the early stages of cardiac arrest.
Compressions are the most important!
If you are doing rescue breaths use the head tilt chin lift technique, pinch the nose close, seal off the mouth, then deliver a normal breath and ensure the chest is rising.
8. Use an AED as soon as one is available.
9. Continue 30 and 2 or hands only cpr until help arrives.
Improvised Chest Seals 👇
Do you always have the correct gear with you 100% of the time? In a perfect world you would but let’s be realistic. Even highly qualified first responders will leave their house to try to live a normal life without all their work items.
Scenario: you just witnessed someone experience a traumatic thoracic injury and do not have a chest seal to cover the sucking chest wound. What can you do? Grab any piece of impermeable plastic and tape it over the hole in the chest and back. You could use a sandwich bag, potato chip bag, plastic wrapper, Saran Wrap etc.
Location: We Seal “the box.” Any traumatic penetrating injury within the navel region (belly button) to collar bone gets a chest seal.
Evidence: There is still some debate in the medical communtiy about whether to tape the improvised chest seal on 3 or 4 sides. If you are going to tape the seal on 3 sides remember to leave the side open that is towards the ground when you roll the patient into the recovery position. This will allow the wound to drain naturally upon expiration and will seal up upon inspiration.
Some new research/guidelines indicates taping on all 4 sides is better because it creates a better seal which is the primary goal. Taping an improvised seal on 3 sides is good in theory but there isn’t a lot of real-world evidence to suggest its effectiveness at properly draining.
Whichever method you choose, practice! No one wants the first time they perform a life saving action to be the real event.

We wanted to wish you all a Happy New Years and start the year off right with a Medical Monday post about what happens to the human body’s blood vessels as it deals with stress and trauma.

While not a new concept, we can assure you that operating in n dual roles as LE/EMS is not an easy task. This can really only be successfully implemented in rural areas with a low EMS call volume and even then it is still not an ideal situation.
We are at an odd time where we are still trying to figure out the best way to handle EMS calls proficiency while maintaining financial concerns and a nationwide staffing crisis.
Should rural EMS be outsourced to other public safety departments? The idea of EMS operating under another department is nothing new.
Whose responsibility is it to ensure first responders are proficient in their duties? 👇
In my humble opinion, I believe both the department/agency and the first responder are responsible for the development of their skills, mental and physical fitness.
Agencies need to be providing their personnel with opportunity to grow. This includes:
1️⃣ Formal/informal Field trainings
2️⃣ Formal training outside of the agency
3️⃣ Time allotted on the job to work out, shoot at the range, train on each other or medical dummy’s etc.
4️⃣ Debriefing after traumatic incidents/ Mental health breaks
First responders need to be motivated to improve their own skills and abilities outside of mandated trainings. This includes:
1️⃣ Working out on their own time
2️⃣ Spending time at the range if your job requires you to carry a firearm.
3️⃣ Developing a new skill (new language, computer based skill, social media skill etc)
4️⃣ Developing their own coping mechanisms to ensure they stay mentally fit.
As an FTO, I could never wrap my head around the idea of giving up on someone unless they have given up on themselves and the job completely. Training may not fix every issue but it can fix many.
There’s no “one size fits all” strategy but If both the agency and the first responder are investing in each other, the outcome should be positive.

Tactical Mindset: Condition yellow v condition red 👇
First responders and military personnel need to operate in “condition yellow” most of the time and occasionally operate in “condition red.”
🟡“Condition Yellow” is when the person is at that perfect balance of a zen like state while maintaining situational awareness and alertness. First responders should strive to live in condition yellow.
🔴 “Condition Red” is the fight or flight response triggered by your sympathetic nervous system. It is an automatic physiological response to a perceived threat.
🧠Soldiers in combat will operate in condition red much longer than civilian first responders. The longer your mind stays in condition red, the higher likely for psychological injury, but in times of combat, is necessary to ensure your survival.
🧠A Swat team point man penetrating the threshold to confront a hostage situation may need to operate in the red to have his cognitive reaction time, visual reaction time, and complex motor skills all operating at peak performance.
🧠Another example would be an EMT dealing with a combative patient.
🧠Yes, you’d lose your fine motor skills, and fall victim to “tunnel vision” but your in a brief state of “fight” which helps keep you safe. If trained, we can reduce the time from visualizing the threat, processing the threat, and reacting to the threat, leading to faster decision making.
🧠Operating in the red is good for brief threatening encounters but we need to switch back to condition yellow as soon as appropriate to ensure we stay physiologically regulated (homeostasis).
🧠This is all about calculated risks and finding that optimal level of physiological arousal for a given task.
💥MARCH: Airway Overview 💥
We wanted to provide a brief overview of airway management options.
🔵Basic airway management is often overlooked. Whether you are an EMT or Paramedic, mastering basic airways is an invaluable skill. Most airways can be managed with simple maneuvers and devices.
🔴The least invasive way to open an airway is using the head tilt-chin lift or jaw-thrust maneuver. Head tilt can be used if there is no suspected spinal injury and jaw thrust can be used if there is a spinal injury.
🟢Simple devices to manage the airway include the oropharangeal (OPA) and nasopharyngeal (NPA) Airways (EMT level). NPAs are used over OPAs in the tactical setting. These basic maneuvers and devices are a great starting point and are also a great safety net if advanced airways fail.
🟡If basic airways are not sufficient, advanced airways can be used like supraglottic airways (iGel, laryngeal mask airway (LMA) King tube etc.), or Endotracheal tube (ETT). ETT is secure but difficult to place. LMA is an alternative to ETT.
🟣Supraglottic airways are typically quicker and easier to place compared to ETT.
🟤Video Laryngoscopes allow for EMS to visualize the glottis for a more successful intubation.
⚪️The goal of all of these devices is to allow a clear path for ventilations and oxygenation.
⚫️Cricothyrotomy is a rare surgical airway procedure used by EMS when conventional methods have failed. This procedure involves an incision in the cricothyroid membrane where an ET tube is inserted. This a last resort procedure used when the provider cannot intubate or ventilate the casualty.
The device/intervention you choose will depend on your training (scope), indications and contraindications, your environment, and resources.
We will cover these in depth in later posts.

The gunman open fired at a bowling alley and bar in Lewingston, Maine killing at least 22 people and injuring dozens.
He should be considered armed and dangerous. Keep an eye out and stay safe!

This appears to be a slash wound of the base of neck. The blood may be dark but there is a strong flow rate.
This type of wound can (and should) be packed with a pressure dressing applied around the wound then wrapped under the armpit on the opposite side.
While many texts books say “don’t pack the neck” this is a generalization due to the airway concerns. While the trachea should NOT be packed, the base of the neck to the shoulder area can absolutely be packed.
This video also shows you what adrenaline can do to keep you alive when you are in that fight or flight mode. A good reminder that the casualty’s mental status may not be an accurate representation of the severity of the injury. Always important to perform your blood sweeps in situations where it may not be this obvious where the bleeding is coming from.

Medical Monday: Hemostatic v regular gauze.
We have talked about the importance of carrying hemostatic gauze (quickclot, celox etc) but we wanted to take a moment to talk about the reality of costs.
Yes, it is true that hemostatic gauze is recommended by us and many other organizations out there. It truly does stop the bleeding 🩸 faster than regular gauze due to impregnated mineral that speeds up the clotting process.
However, the 2 most important factors when talking about wound packing are:
1️⃣ Pinpoint direct pressure on the source of bleeding (severed artery).
2️⃣ Packing the wound completely to ensure adequate pressure is maintained.
We understand the costs associated with purchasing hemostatic gauze can be around $50 💰 for 1 package and the costs for purchasing regular gauze are about 1/20th the cost.
Why are we telling you this?
Because if you have to make the financial decision between 1 pack of hemostatic gauze or several packs of regular gauze it may be a smart consideration to purchase several regular gauze packets compared to 1 pack of combat gauze. The video and image is to show you how much gauze needs to be packed in even the smallest of wounds.

TACMED ACADEMY is proud to be a recognized educational partner of the Committee for Tactical Emergency Casualty Care (C-TECC)
This means that we agree to teach guidelines and medical information based on their principles. While we follow their guidelines, we provide an even deeper dive into tactical medicine.
TECC was developed based on the guidelines set forth in the Committee on Tactical Combat Casualty Care (CoTCCC). CoTCCC was developed in response to the alarming number of preventable deaths occurring on the battlefield. We have seen a dramatic reduction in preventable deaths since it’s implementation for 2 main reasons:
1. Proper evidence based education.
2. Availability of life saving medical equipment.
The two committees are very similar but TCCC was specifically designed for the military population and TECC was developed for casualties infant to elderly.
We saw the need for education with those low frequency/high risk calls in the civilian world because whether someone suffered a traumatic injury in the Afghanistan or Boston, Massachusetts, the care is surprisingly similar.

Su***de rate have been climbing for all Americans but police officers (and other first responders) tend to get hit harder.
Their chronic exposure to death, trauma, and crisis makes them especially vulnerable.
Of the 18,000 police agencies around the country, fewer than 5% have su***de prevention programs for their officers
Since 2016, more law enforcement officers have died by their own hands than killed in the LOD.
From 2016 to 2020, over 900 officers su***des were reported to blue help (organization for LEOs).
Vicarious trauma (secondary trauma) is a result of continued exposure to victims of violence and trauma in addition to witnessing pain and fear that others have experienced.
Dr. John Violanti and his colleagues conducted research on the overall health of officer and their results indicated that illnesses among police officers were more prevalent than the general population.
Shift work and chronic stress lead to panic attacks, PTSD, alcohol/substance abuse, depression and suicidal ideations. The stress produces psychological and physiological reactions that could overcome a healthy officers ability to cope.
Are we saying you cannot drink any alcohol? Of course not. We just want you to recognize when you are drinking more than you usually do so you can know when something is off.
Breathe, process, then respond.
What does your agency (fire, EMS, or law enforcement) do or what system do they have in place if an employee sought help?

On July, 28th NH took a huge leap forward in the care EMS provides. Governor Sununu signed and passed legislation (Max’s Law) that allows EMS to treat and transport canines working in the line of duty. EMTs will now be given immunity from personal liability for transporting the dog (unless there are signs of gross negligence).
The law change comes from the family of Yarmouth MA K9 Sgt Sean Gannon who was killed in the line of duty. His partner Nero suffered a gun shot wound from the same incident and was not allowed to be treated or transported by EMS on scene due to no protocols in place. There were even empty ambulances on scene that could have assisted.
The family of Sean Gannon pushed for the law change in MA and now have successfully implemented it in NH. The next step is to pass a set of official protocols and procedures for handling K9’s injured in the line of duty.
Nero did survive despite being shot in the head due to an officer transported him to a vet in the back of his cruiser.
We believe this is a common-sense law that should be enacted in all 50 states. K9’s are a part of the family/workforce and need to be treated with the utmost respect and medical care you would a human.
Well we certainly don’t teach this…
⚠️⚠️⚠️ Tale of Don’t Do THIS from
Patient went to a "traditional healer" for snakebite treatment. A tourniquet was applied and left on for a whopping 14 days! The photo shown above was the result.
2 weeks before going to the hospital, this patient had felt a sudden sting on his left thumb while packing a heap of grass in his farm and realized he had been bitten by a snake. Within a few minutes, he started having abnormal sensations in his hand. There was a gradual onset of swelling which progressed from the left hand to mid-arm. He also had palpitations, and shortness of breath. He sought help from a traditional healer who applied a multi-threaded jute tourniquet over his left elbow and some local herbs over the bite site.
Days after tourniquet application, he noticed blackening of his fingers. It progressed to involve his left hand and forearm up to the site of the tourniquet. Due to difficulty in accessibility, it took another week for the patient to present to the hospital.
On examination, the left hand and forearm was black in color, and foul smelling. There was ulceration over the site of tourniquet application, cold on touch, radial pulse was absent, axillary lymph nodes were not palpable and muscles were mummified.
His pulse was 94 bpm, blood pressure was 100/70 mmH, and he had a fever. There was wet gangrene of the left upper limb below the elbow. After ortho consultation, he received an above elbow amputation and was admitted to the hospital for recovery.

🩸Extremity Hemorrhage control for K9🩸
Man’s best friend.
Almost 50% of American households have dogs. There are thousands of dogs working in the military and civilian first responder world.
A dog that loses more than 30% of its blood is at risk of shock and eventually death.
Basic management of extremity bleeding control includes:
1. Muzzle dog to prevent them from biting. It does not anther how friendly the dog is, an injured animal is always unpredictable.
2. Apply direct pressure to the limb using gauze, a towel, or cloth.
3. Reapply more material if the blood is soaking through. Don’t remove the first layer because it may have started the natural clotting process (thrombus formation) already.
4. Use a pressure bandage or cloth to wrap firmly around the wound to hold direct pressure.
5. If the bleeding is bright red high pressured blood (arterial), apply either a K9 tourniquet or wrap a bandage around the dogs limb as high as you can get it. You can usually control the bleeding with a tightly wrapped bandage due to the smaller size of their limb.
6. Dogs are anatomically different than humans. Don’t use a CAT or SOFT on a dog because it will likely slide down their leg due to its tapered shape.
7. Get the dog to an animal hospital as quickly as you can.
8. Monitor the dog for signs of shock to include fast heart rate, weak pulse, pale gums etc.

What is Combat Gauze and is it recommended? 👇
Combat Gauze is our preferred hemostatic gauze. It is an essential component to add to your trauma kit in addition to plenty of regular gauze in your kits too.
Combat Gauze is a CoTCCC approved hemostatic wound dressing designed to aid in the cessation of bleeding.🩸
Combat Gauze is impregnated with kaolin, an inorganic mineral that activates factor Xll (Hageman Factor).
Factor Xll is a plasma protein involved in the coagulation cascade. Factor Xll initiates intrinsic pathway to assist the formation of fibrin (protein) and thrombin (enzyme) to form a clot.
The goal of packing with combat gauze is to assist the casualty with their natural clotting ability (coagulation cascade) to achieve hemostasis.
Combat gauze contains an embedded X-ray strip so the ER 🏥 will (should) know the gauze has been packed inside the casualty. It comes Z-folded, sterile, and vacuum packed for easier storage/application.
While Combat Gauze should not be overly relied upon, it is an essential item to keep with your regular gauze.

Windlass style tourniquets are superior to those that just supply circumferential pressure because of the mechanical leverage. ⚕️The combination of the circumferential pressure AND the windlass use is what makes tourniquets as successful as they are. ⚕️Did you know that while there were 27 improvised tourniquets made during the 2013 Boston Bombing, none had a windlass created. RATS TQ breakdown: 🔵We poke fun but in all honesty the RATS tourniquet isn’t a horrible option to carry if it’s just a backup (something is better than nothing) but it should never be the tourniquet you rely on as your first option.⚪️Studies have shown that the RATS TQ can be effective at hemorrhage control but is not as effective as other commercial tourniquets such as the CAT and SOF. 🟢The RATS TQ will control massive hemorrhaging 🩸but it has been shown to be take longer to completely occlude blood flow compared to CAT and SOF. The RATS is more difficult to get consistent circumferential pressure and it was shown to have a longer application time. 🟡The RATS has a bungee style band that is thin and does not offer a windlass. We are are a huge fan of windlass style tourniquets because the evidence does not lie at their success. After all, the RATS TQ is not CoTCCC recommended for a reason. 🟣People usually carry the RATS as an EDC because of how compact it can get. If you truly need a smaller edc tourniquet we would gravitate towards Snakestaff Systems because it has a much better design that we can support.⚠️Disclaimer⚠️: we have not done clinical testing on either TQ so we only offer you our opinion based on training and experience. ⚠️Warning graphic video ⚠️ This appears to be a slash wound of the base of neck. The blood may be dark but there is a strong flow rate. This type of wound can (and should) be packed with a pressure dressing applied around the wound then wrapped under the armpit on the opposite side. While many texts books say “don’t pack the neck” this is a generalization due to the airway concerns. While the trachea should NOT be packed, the base of the neck to the shoulder area can absolutely be packed. This video also shows you what adrenaline can do to keep you alive when you are in that fight or flight mode. A good reminder that the casualty’s mental status may not be an accurate representation of the severity of the injury. Medical Monday: Hemostatic v regular gauze. We have talked about the importance of carrying hemostatic gauze (quickclot, celox etc) but we wanted to take a moment to talk about the reality of costs. Yes, it is true that hemostatic gauze is recommended by us and many other organizations out there. It truly does stop the bleeding 🩸 faster than regular gauze due to impregnated mineral that speeds up the clotting process. However, the 2 most important factors when talking about wound packing are: 1️⃣ Pinpoint direct pressure on the source of bleeding (severed artery). 2️⃣ Packing the wound completely to ensure adequate pressure is maintained. We understand the costs associated with purchasing hemostatic gauze can be around $50 💰 for 1 package and the costs for purchasing regular gauze are about 1/20th the cost. Why are we telling you this? Because if you have to make the financial decision between 1 pack of hemostatic gauze or several packs of regular gauze it may be a smart consideration to purchase several regular gauze packets compared to 1 pack of combat gauze. The video and image is to show you how much gauze needs to be packed in even the smallest of wounds. On July, 28th NH took a huge leap forward in the care EMS provides. Governor Sununu signed and passed legislation (Max’s Law) that allows EMS to treat and transport canines working in the line of duty. EMTs will now be given immunity from personal liability for transporting the dog (unless there are signs of gross negligence). The law change comes from the family of Yarmouth MA K9 Sgt Sean Gannon who was killed in the line of duty. His partner Nero suffered a gun shot wound from the same incident and was not allowed to be treated or transported by EMS on scene due to no protocols in place. There were even empty ambulances on scene that could have assisted. The family of Sean Gannon pushed for the law change in MA and now have successfully implemented it in NH. The next step is to pass a set of official protocols and procedures for handling K9’s injured in the line of duty. Nero did survive despite being shot in the head due to an officer transported him to a vet in the back of his cruiser. We believe this is a common-sense law that should be enacted in all 50 states. The Body cam footage has just been release for the Allen, Texas Mall shooting that occurred on May 6, 2023. The gunman killed 8 people including 2 elementary school aged children. At first, you may think the officer wasn't moving quickly but you have to remember that he didn't know where the gunman was and it can be very difficult to tell where the gunshots are coming from. Gunshots will echo and the best you can do is figure out the general direction they are coming from. As you can tell by the video, this officer was conducting normal patrol duties when this happened. He goes from ensuring the children are strapped to getting strapped himself. You never know when you could be near an incident like this. Shows you how important it is to be trained and have the right gear with you. Medical Monday! ⚠️ (Sucking Chest Wound) ⚠️ This video depicts a real “sucking chest wound” (open pneumothorax). An SCW is caused by penetrating trauma (gunshots, stabbings etc.) to the chest wall. In order for a SCW to form, the hole in the chest wall needs to be large enough that the skin won’t seal it up (around the size of a penny or larger). The “sucking” noise is caused by an audible passage of air 💨 during both inhalation and exhalation. The hole could make a bubbling noise if air passes through blood.🩸 SCW are concerning but not nearly as fatal as tension pneumothoraxes. A tension pneumothorax can form when air enters the thoracic cavity (pleural space) but cannot escape. In the field, the easiest medical intervention to treat a SCW is to cover the hole with a gloved hand, then apply a commercial chest seal like the North American Rescue Hyfin. We will cover: 1. Who brings a trauma kit to the range with them? 🙋🏽♂️ We are strong believers that if you own firearms or go shooting, you need to have the right gear with you and more importantly, know how to use it. Firearm accidents occur all the time due to a lack of training or negligence. Be that guy that is always prepared 💪🏼. Always hope for the best, but be prepared for the worst. At a minimum your range kit should include: 1️⃣ Tourniquets (We prefer NAR CAT TQ) 2️⃣ Vented Chest Seals (We prefer NAR Hyfin) 3️⃣ Hemostatic Gauze (We prefer QuikClot Combat) 4️⃣ Regular Gauze (We prefer Z-folded Compressed) 5️⃣ Pressure dressing (We prefer Olaes Bandage) As always, make it home to your family at the end of the day!
If you own or work with dogs read this post 👇 Man’s best friend. Almost 50% of American households have a dog. There are thousands of dogs working in the military, law enforcement, on farms. As incredible as dogs are, what is something they cannot do? PROVIDE SELF AID IF INJURED. There are 3 types of bleeding: 1. Capillary 2. Venous 3. Arterial Life threatening bleeding is arterial but venous bleeding can pose a significant threat too. A dog that loses more than 30% of its blood is at risk of shock than eventually death. Basic management for K9 extremity bleeding control: 🩸 1️⃣ Muzzle dog to prevent them from biting. It does not matter how friendly the dog is to you. If it’s injured, you are at risk of getting bit. 2️⃣ Apply direct pressure to the extremity using gauze, a towel, or cloth. 3️⃣ Reapply more material if the blood is soaking through. Don’t remove the first layer because it may have started the natural clotting process already. 4️⃣ Use a pressure bandage or cloth to wrap firmly around the wound to hold direct pressure. 5️⃣ If the bleeding is bright red arterial bleeding, apply either a K9 tourniquet or wrap a bandage around the dogs limb as high as you can get it. You can usually control the bleeding with a tightly wrapped bandage due to the smaller size of the limb. 6️⃣ Dogs are anatomically different than humans. Never use a CAT or SOFT-T tourniquet on a dog because it will slide down due to their tapered limb. There are tourniquets designed specifically for dogs. 7️⃣ Get the dog to a vet as quickly as you can. 8️⃣ Monitor the dog for signs of shock to include fast heart rate, weak pulse, pale gums etc. Comment below if this was helpful! ✅

First responders are almost 5x as likely to be involved in a fatal crash due to the nature of the response. In addition to driver training, you should set time aside to practice a lifesaving skill: applying a tourniquet to yourself if you are involved in a motor vehicle accident. Try out different drills like: 1️⃣ 1 handed application for your arm 2️⃣ 1 handed application for your leg (pictured) 3️⃣ 2 handed application for your leg Understand that there will be challenges to tourniquet application when involved in a crash due to: 1️⃣ Stress 2️⃣ Injuries 3️⃣ Air bag deployment 4️⃣ Visibility issues 5️⃣ Crushed in dashboard/bent metal 6️⃣ Steering wheel The tourniquet in your trunk won’t do you any good. Keep a tourniquet somewhere easily accessible to you while you drive. Some good place to keep a tourniquet is: 1️⃣ In a cup holder 2️⃣ Attached to the visor 3️⃣ Attached to head rest 4️⃣ In the center console Make it home to your family at the end of every day!

A great story about how it pays to be prepared! You do not need to be a first responder, special ops, or a medic to save a life. It’s not about looking “tacticool” or having all the flashy toys (although mandatory if you’re SWAT 😉) All you need is some basic trauma training and carry the right gear with you. If you don’t want to do it for strangers, then at least do it for your family. Anyone can get trained in tourniquets. If you are a parent, than this should resonate with you. Anyone’s child could be involved in that crash. The “tactical environment” that we always talk about isn’t just active shooter or mass casualty scenarios, it could be as simple a bad car crash.
