How Parathyroid Gland Surgery Is Performed?

Parathyroidectomy is a surgical procedure to remove abnormal or diseased parathyroid glands. Humans have four oval-shaped parathyroid glands located behind the thyroid glands in the neck. Parathyroid glands produce special hormone called parathyroid hormone or PTH. This hormone regulates calcium level in the blood which is essential for proper functioning of body organs.

Why is parathyroidectomy performed?

Due to certain reasons, if our parathyroid glands overstimulate or overact, they develop into a condition known as hyperparathyroidism. In this condition, abnormal parathyroid glands produce and release high levels of PTH hormone even when it is not required. Therefore, hyperparathyroidism must be treated immediately by removing diseased or abnormal parathyroid gland and this can only be done surgically through a procedure called parathyroidectomy.

Why does hyperparathyroidism occur?

Hyperparathyroidism occurs due to tumor growth in the parathyroid glands or vitamin D deficiency in our body. Due to unknown reasons, if tumor grows in our parathyroid glands, it causes abnormal division and proliferation of parathyroid cells and results in enlargement of parathyroid glands. The enlarged parathyroid glands will overact and produce abnormally high levels of PTH hormone. This leads to the development of hyperparathyroidism. In other case, because vitamin D is responsible for absorption of calcium from intestine into the blood. Therefore, if vitamin D deficiency occurs in our body, calcium will not be absorbed into the blood and blood calcium level falls critically. This stimulates parathyroid glands which continuously grow and become enlarged to cope the low blood calcium level. As a result, hyperparathyroidism occurs which needs immediate removal of abnormal parathyroid glands through a surgical procedure known as parathyroidectomy.

How is parathyroidectomy performed?

Before surgery, doctors examine test reports of few blood tests including complete blood count or CBC etc.,
During the procedure, patients typically receive general anesthesia to become unconscious during the surgery. Once the patient completely sedate, surgeons make about half-inch incisions in two sides of the neck and use special retractors to approach parathyroid glands without damaging the nearby tissues. Through one incision, an endoscope with camera in front is inserted to visualize the diseased parathyroid glands which are then removed with the help of instruments inserted through the second incision. Once, surgeon ensures that all the diseased parathyroid glands are removed, he sutures the subcutaneous layer and fix the top skin layer using surgical glue.
Parathyroidectomy is mostly applied in outpatient settings means that patients do not have to stay overnight in the hospital, even they are discharged within two to three hours of surgery.

Birthmarks - How Birth Marks Develops In Our Skin?

Birth marks are the marks which appears on our skin before or just after the birth. Birthmarks can vary in size, colour, and shape. They usually appear on our head, neck and face.
Mostly they disappear with time like moles, However, some birthmarks develop slowly and appear on our skin throughout our lives.

Causes of birthmarks
The exact cause of birthmark is unknown, However, by observing pigmented and vascular birthmarks, we can gain an understanding of how birthmarks form.

Pigmented Birthmarks
Pigmented birthmarks form as a result of the pigments that give our skin colour. Our skin contains melanocyte cells, that produces melanin pigment. Melanin give color to our skin. If melanin production is disrupted for any reason, skin color will appear different than in other places.
This is known as a pigmented birthmark.

The main cause of pigmented birthmark is the accumulation of melanocyte in skin, abnormal production of melanin due to some genetic mutation, or a melanocyte trap in the dermis layer.
If the number of melanocytes in a particular area of our skin increases, Then production of melanin increases in that area, which cause change in color of skin. If mutation in DNA of melanocytes occurs, this cause production of melanin more or less in affected area. Due to which skin color can change and cause birthmarks.
Similarly, DNA mutations during foetal development can cause melanocytes to become trapped in the dermis layer. This can cause birthmark in affected area.

Vascular Birthmarks
Vascular birthmarks forms due to abnormalities in our blood vessels E.g Hemangiomas, salmon patches, or port wine stains.

Hemangiomas
These birthmarks forms due to abnormal development of blood vessels (arteries). The skin color appear red in hemangiomas. According to some scientists, hemangiomas are caused by abnormal protein formation during foetal development.

Salmon Patches
Sometimes our small blood vessels (capillaries) accumulate abnormally beneath our skin's surface. which cause skin color red or pink in affected area. These birthmarks are called salmon patches.

Port Wine Stain
Our blood produce some special chemical signals, which control blood flow. If these chemicals form abnormally as a result of gene mutation, blood flow in some areas may be disrupted. Due to which our skin color changes to red, and cause birthmark. This type of birthmark is called port wine birthmarks.

Treatment
Pigmented birthmarks may disappear with time, But in some cases these birthmarks are removed surgically, like Giant Moles. Vascular birthmarks are treated by different ways, which include cosmetic solutions, makeup, medicines, laser treatment and surgery.

Why ECG (Electrocardiogram) is performed?

ECG, or electrocardiogram, is a test that detects our heartbeats. This test includes three main instruments: electrodes, wire, and an ECG machine. Electrodes are attached to various parts of our chest, arms, and legs, which are linked to an ECG machine via wires. The lengths of ECG waves are denoted by the letters P, Q, R, S, and T. P, Q, R, S, T letters are used fro the convenience. These waves help us to understand heartbeat easily.

Why ECG Test is performed?
It is used to detect various heart defects. These heart defects include SA node malfunction, slow or fast heartbeat, heart attack, pacemakers and medication that disrupts heart rhythm

Sino-Atrial Nodes
The SA node, which is located on the right side of the heart, produces mild electric impulses. These impulses control our heart contraction and relaxation. If they do not function properly, our hearts do not contract and pump properly. As a result, our heartbeat becomes abnormal. This condition can be detected with ECG.

Why our heartbeat get slow and faster?
ECG can detect abnormally fast or slow heartbeats caused by exercise or stress. Because these conditions are not continues. So, detecting them with a standard ECG is difficult. Theses conditions are detected by event monitor devices. The event monitor is a portable ECG device that is attached to the patient's chest. This device is activated by displaying heart symptoms caused by stress and exercise. It records electric impulses, and it is usually placed for 30 days

Heart Attack
If arteries in our heart gets blocked, this cause severe pain in our heart, which can lead to heart attack. This type of conditions can be detected with ECG. So we can find blood flow in our different parts of heart, and detect blockage in the heart.

Implanted Pace makers
Pacemaker devices are implanted in patients with slow heartbeat. These devices generate mild electric impulses, and make our heartbeats normal. Because the devices have been implanted for a long time, an ECG test is performed on a regular basis to detect their performance.

Heart medication
Medications used to treat heart defects or medications that have the potential to cause heart problems, The ECG is then performed to detect any adverse effects of that medications on the heart.

Treatment
If an abnormal heartbeat is detected on an ECG, the test is usually repeated. Also, an echo-cardiogram is performed to determine the exact cause of the abnormal heartbeat, and then treat the patient as needed.
