[Review] Poncet's disease (tubercular rheumatism): a narrative review


Poncet's disease or tubercular rheumatism is uncommon, and there are not many related studies. No consensus has been achieved with respect to its etiopathogenesis. The low prevalence of this Poncet's disease even in endemic regions suggests hidden mechanisms that require further research to fully comprehend this enigmatic disorder. Careful identification of primary focus of tubercular infection, clinical suspicion and knowledge of this disorder is crucial for prompt identification and treatment. This article aims to provide an overview of the recently published articles about Poncet's disease. The article attempts to highlight key features of the disease for better understanding of this peculiar disorder.

[Study Protocol] Clinical and morphological characteristics of chronic inflammation in the myocardium in patients with decompensated chronic heart failure accompanied by ischemic systolic dysfunction: study protocol for an open-label nonrandomized trial


Heart failure is one of the leading causes of morbidity and mortality in the world. Mortality remains high (50%), despite optimal treatment according to the recommendations of the European Society of Cardiology and Russian Society of Cardiology. Tremendous evidence has emerged over the past decades that heart failure is associated with inflammation. However, anti-inflammatory therapy widely lacks positive outcomes. Currently, there are no clinical studies investigating the molecular and cellular phenotypes of inflammation in myocardial tissue (viral, viral and autoimmune, and autoimmune types of inflammation) in patients with decompensated chronic heart failure (CHF). Therefore this study aims to develop a new medicinal drugs and approaches to treatment of decompensated CHF.

Chondromyxoid fibromas are rare, benign tumours accounting for < 1% of primary bone neoplasms, and most commonly affected in the second or third decades of patient's life. We reported one such case of chondromyxoid fibroma in the distal fibula of a 15-year-old girl. The patient was managed with lower 3 rd fibulectomy & fibular turnoplasty from middle 3 rd fibula with one-third tubular plate fixation for stabilization followed by bone grafting. The patient is disease-free since 3 years.

[Study Protocol] Assessment of coronary flow reserve by Doppler flow wIre in patients with acute coronary syndrome undergoing percutaneous coronary intervention: differences between the LoAding dose of Ticagrelor versus prasugrEl (DILATE): study protocol for a randomized controlled trial


Percutaneous coronary intervention (PCI) is the main treatment in patients with acute coronary syndromes (ACS). Current guidelines recommend the administration of a loading dose of clopidogrel, ticagrelor or prasugrel during the procedure. P2Y12 antagonists increase adenosine concentration by inhibiting red blood cells reuptake and induces adenosine triphosphate release from human red blood cells, which is further degraded to adenosine. Ticagrelor has been demonstrated to increase the coronary blood flow velocity more than prasugrel, as assessed by transthoracic Doppler echocardiography after incremental doses of adenosine administration. Currently, no data are available on comparison between prasugrel and clopidrogrel in term of vasodilation effect evaluated invasively.

The objective of the current study is to determine the efficacy of spontaneous laughter in improving the post-operative prognosis for pediatric patients after minor surgery.

Traction apophysitis of the accessory ossification center of the medial malleolus is infrequently reported. Traction apophysitis of both the accessory ossification centers of medial malleoli is very rare. This entity is also reported as osteochondrosis of medial malleolus in literature. We present a case of traction apophysitis of accessory ossification centers of both the medial malleoli in an 11-year-old boy of Indian origin, who presented with pain and swelling over both the ankle with plano-valgus deformity of feet. His imaging revealed fragmented accessory ossification center of medial malleoli on both the sides. He was treated non-operatively and followed through one year to see the eventual fate.

[Review] Anxiety sensitivity and substance-related disorders: a narrative review


Anxiety sensitivity means fear of anxiety-related sensations which is one of the important factors in the development and maintenance of substance-related disorders. Studies support the underlying, mediating, and moderating role of this construct in the etiology of substancerelated disorders. This review suggests that anxiety sensitivity can play a role in this group of disorders in different ways, such as fear of body sensations, fear of cognitive dyscontrol, and fear of socially observable anxiety symptoms. Finally, high anxiety sensitivity can interfere with the treatment of substance-related disorders as an underlying vulnerability and increase the likelihood of relapse in this group of people.

Transforaminal endoscopy has certain advantages over traditional surgeries (e.g., intervertebral disc fusion) in the treatment of lumbar disc herniation, as it is associated with minimal trauma, clear surgical vision, and lower degree of bleeding. However, the therapeutic effects of transforaminal endoscopy on lumbar disc herniation in human immunodeficiency virus (HIV)-infected patients are poorly understood. The purposes of this study are to indicate the efficacy of transforaminal endoscopy in the treatment of lumber disc herniation in HIV-infected patients and compare the efficacy between transforaminal endoscopy and conventional lumbar disc fusion based on 2-year follow-up and prognosis turnover.

Schizophrenia is associated with excess mortality primarily due to cardiovascular disease. Unhealthy lifestyle and sideeffects from pharmacologic treatment are among the risk factors that highly contribute to somatic disease in patients with schizophrenia. While the number of cardiac deaths is declining in the general population, the cardiac mortality in patients with schizophrenia remains high. No other studies have so far investigated the presence and progression of arteriosclerosis in patients diagnosed with schizophrenia. The primary aim of this clinical study is to reduce the high rate of cardiac mortality in patients with schizophrenia.

Scalp block is a selective nerve block technique to inhibit the afferent input from nerves that innervate the scalp, including supratrochlear, supraorbital, zygomaticofacial, and zygomaticotemporal nerves for the anterior scalp. Skull pin insertion is known as the strongest stimulus prior to craniotomy. The results from previous studies are inconclusive regarding the advantage of scalp block as a pre-emptive analgesic technique. This study will fill this knowledge gap, and examine whether this technique is worth performing as a standard technique.

[Correspondence] Administration of tranexamic acid in primary total joint arthroplasty


One of the concerning problems with total joint arthroplasties is a need for postoperative blood transfusion. Currently, evidence is gathered that allogenic blood transfusion has more disadvantages than previously thought. In the search for factors that may contribute to influencing the need for blood transfusion, tranexamic acid has been tested. Currently, it has a well-documented effect on reduction of intra- and postoperative blood loss and transfusion requirements in patients undergoing total knee arthroplasty. In this brief review, we will discuss documented use of tranexamic acid in total joint arthroplasties, focusing on the hip joint.

[Study Protocol] Effects of a combined nerve block on intraoperative stress and postoperative immune function in elderly patients subjected to total hip replacement: study protocol for a randomized controlled trial


Anesthesia, surgery, and other nociceptive stimuli affect stress and hemorheological indices, impact physiological function, decrease immune function, and thereby influence recovery of hip joint function in elderly patients who undergo total hip replacement. Previous anesthesia methods for hip replacement in elderly patients include general, lumbar puncture, or epidural anesthesia alone. A combined nerve block is more suitable for total hip replacement in the elderly because of the safety and reliability of the method. In this study, we hypothesized that a combined nerve block is superior to any previous anesthesia method alone for total hip replacement in the elderly. Specifically, we hypothesized that intraoperative stress, hemorheological indices, postoperative immune function, and incidence of postoperative complications would be more favorable using a combined nerve block compared with previous anesthesia methods.

[Study Protocol] Feasibility of implantation of a cemented femoral stem in the treatment of osteoporotic femoral neck fracture in elderly patients: study protocol of a randomized controlled trial


Osteogenesis is a common problem after surgery for femoral neck fracture in elderly patients. Internal fixation for the treatment of femoral neck fracture should be performed to optimize bone remodeling and strengthen fractured bone trabeculae, with the aim of achieving strong fixation from the perspective of biomechanics. Percutaneous internal fixation with cannulated compression screws has become a preferred treatment method of osteoporotic femoral neck fracture in elderly patients, but the insufficient holding power of the screws used in the femoral neck does not lead to strong fixation. An alternative, joint prosthesis is recommended for the repair of femoral neck fracture in elderly patients. However, its long-term therapeutic effects in the treatment of osteoporotic fracture of proximal femoral neck remain poorly understood in patients with avascular necrosis of the femoral head.

As a biological marker of alcohol misuse, the specificity of the CDT is known to be superior to GGT and MCV as well. The elderly may suffer from this pathology, but its clinical and biological detection and management are often difficult, due to a frequent denial of the alcoholism and multiple chronic diseases.

Cortical spreading depression (CSD) is an important pathological mechanism behind migraine. Electrical fastigial nucleus stimulation can reportedly inhibit the occurrence and propagation of CSD, and therefore can be used to prevent migraine.

[Study Protocol] Porous tantalum rods improve the hip joint function of patients with avascular necrosis of the femoral head after femoral neck fracture surgery: study protocol of a randomized controlled trial


After femoral neck fracture, insufficient blood supply to the femoral head can cause avascular necrosis of this region. This can lead to increased treatment difficulty, and may greatly influence the recovery of hip joint function. Core decompression with bone grafting is a common treatment for avascular necrosis of the femoral head. An implanted porous tantalum rod, which is a bone trabecula-like metal implant that is used to support the weight-bearing area of necrotic bone, can prevent further collapse of the necrotic area. This strategy has led to favorable effects in the early treatment of avascular necrosis of the femoral head. We hypothesized that, compared with core decompression alone, core decompression with porous tantalum rod implantation would have a more beneficial effect of the hip joint function of patients with avascular necrosis of the femoral head after femoral neck fracture surgery.

The root cause of intertrochanteric femoral fracture in elderly patients is osteoporosis manifested by reduced bone strength, decreased bone mineral density, and subsequently decreased anchoring between the internal fixator and bone. This is also a major cause of ineffective internal fixation and leads to surgical failure. Because of the special anatomical structure and biomechanical requirements of the proximal femur, internal fixation must have two basic functions, to prevent inversion and rotation of the proximal fractured femur. Reducing the incidence of coxa vara is also one of the treatment objectives.

[Study Protocol] Polymerase chain reaction for diagnosis of isolated tuberculosis of the wrist joint


A 43-year-old female patient complained of pain, morning stiffness and mild swelling in the right wrist joint for 6 months. She had no history of any constitutional symptom, weight loss or involvement of other joints, but had suffered from pulmonary tuberculosis for 10 years and had completed a full course of directly-observed treatment short course. Nonresponsive management of inflammatory arthritis dragged us to use a highly sensitive test for tuberculosis, that is, polymerase chain reaction (PCR), and PCR findings from this patient were positive. This experience led us to conclude that even though conventional stain for acid fast bacilli comes to be negative, a more sensitive and specific test is needed for better diagnosis at earlier stages of the disease.

[Study Protocol] Cortical physiology as a therapeutic target in Parkinson's disease related dementia and cognitive dysfunction: study protocol for a randomized controlled trial


Dementia is the leading cause of nursing home placement in Parkinson's disease (PD), yet little is known about the causes of cognitive dysfunction in PD and there are no effective treatments. Our preliminary data and other published studies suggest that abnormalities in brain activity involving networks important for normal thinking and memory may contribute to cognitive dysfunction in PD and may represent a target for treatment.

Acupuncture is a relatively safe treatment for pain, and its analgesic effects have been confirmed. Electroacupuncture (EA) has been widely used to treat migraine because of its continuous and highly controllable stimulation. However, few rigorously designed randomized controlled trials have evaluated the efficacy of EA at the Hegu and Taichong acupoints in the treatment of migraine.

[Study Protocol] The best modern practices to reduce surgical complications after total hip and knee replacement


Although there is the use of pharmacological prophylaxis to prevent venous thromboembolic diseases after total hip or knee replacement, venous thromboembolism is considered one of the main and serious complications. Moreover, the incidence of deep venous thrombosis among the patients who have total hip or total knee replacements is still high. The use of regional anaesthesia in arthroplasty has been increasing recently. In addition to its effects in elimination pain, regional anaesthesia may help to reduce the incidence of postoperative thromboembolism. Given this, this paper aims to highlight the role of regional anaesthesia to improve outcomes of hip and knee arthroplasty.
