Join me for a look at the use of CBCT in the diagnoses of a transverse discrepancy, and if it offers the solution to the debated topic. This podcast is based on a lecture by Chun Hsi Chung at this year’s AAO. The conventional methods of assessment,Curve of Wilson and WALA ridge line, are reviewed through the lens of a CBCT, as well as how to use a CBCT to assess the maxilla and mandible, which although revealed an ideal measurement, may not be telling the full story.

“To conclude with I wanted to describe to you the American Academy of Dental Sleep Medicine's consensus statement from this year And it looks at the treatment modalities in orthodontics starting off with expansion There was no consensus whether it could either prevent or be management for obstructive sleep apnea There's difference of opinions There is evidence that's there Interestingly there isn't enough evidence to state if it could be a cure for it When it came to mandibular advancement there's even less information and it's unclear if it can even carry out preventative measures be involved with management and indeed be curative”

“So what are my thoughts on Lightforce It's exciting to see this digital integration into labial conventional appliances CAD-CAM technology is not new to orthodontics but has so far been really the preserve of clear alignotherapy and customised lingual appliances It's great to see these ideas and concepts being brought into conventional labial fixed appliances The items that have been described with Lightforce are not necessarily unique to that system For example the idea of offsetting bracket positioning that has been established in lingual appliances But it can be often difficult to get the brackets into the ideal place Digital planning and visualisation is part and parcel of how clear align therapy works And it was actually there with the inception of clear aligners There's also this idea of customisation At what point do we then look elsewhere bracket is one component but we've still got to look at the archwise as well And I tried to find Lightforce's recommended archwise sequence and couldn't find it And that is a significant part of the customisation of force delivery”

Why does tooth movement-related bone loss not necessarily cause gingival recession? SNIPPET

“The next question that Greg Wangs posed was does bone loss result in gingival recession The presumption is that the answer should be yes That's not what Greg found He cited an example a patient having 3 millimetres of bone loss occurring according to the CBCT but actually far less gingival recession Now when it came to looking at previous studies what he found was that Kim's study from 2024 showed significant palatal bone loss on patients having retraction of their incisors by the use of mini screws Now when these patients were followed up long term in retention it was found the palatal bone actually recovered And this really starts to conflict with the idea that bone is needed to maintain the gingiva And also does that mean we are getting new bone formation or is this a different type of bony remodelling occurring What Greg did state was how he thinks this happens but it needs further And although the alveolar bone may disappear the periodontal ligaments and the periosteum are still present and they both have osteogenic potential So if tension does occur between periodontal ligaments and the periosteum new bone formation may well occur This may well explain why bone loss itself doesn't necessarily correlate to recession occurring as well”

“What was the bone loss that they found Well interestingly enough when they expanded in the maxilla there was no real difference between both the expansion and the non-expansion aligner groups And this was found to be minimal bone loss across both It was the mandibular arch where significant differences were found Now in the expansion group there was 1 millimetres of mandibular bone loss across the central and sides region When it came to looking at other areas of the arch there was a one point four millimetres of bone loss across the premolar region What about movements that took place to the incisors There was very little change for the inclination of the maxillary incisors Again the mandible seems to be where significant changes happen There was significant tipping of those lower incisors”

“One study you know Larson in in 2015 they had over five thousand People I mean seriously they had premolars extracted no premolars extracted followed up And the group that had extractions had ten percent of OSA prevalence incidence that ten percent of that population had OSA and the non-extraction group had ten percent roughly So there wasn't any difference So that cannot definitively show But then you'll ask the airway advocates and they're like oh no you haven't seen these They must have done this and they've selected this How come they're so quick to criticize those studies but they get their feelings hurt when we use the same template they use on us on their studies where n equals 4 right We show a study of N equals 5000 Not enough Problems with that study We talk about their study of n equals four and it's oh my God it's you know we're being unfair I think with this whole extraction debate it doesn't I think people shouldn't have to even talk about this extraction issue being an issue when it comes to OSA”

“So let's talk about myofunctional therapy Talk about myofunctional therapy for speech swallowing advancing the position of the tongue growing the mandible correcting the growth pattern of the mandible That's what it said in that paper And the whole point of this myofunctional therapy is to strengthen the muscles of the tongue the lips and all of that which is great That's the basis behind it Then the child falls asleep is a state of unconsciousness So the muscles of the face and mouth are not under voluntary control So how does that impact one's OSA Now the studies show to some degree it has an impact they have difficulty dealing with the other confining variables and the other issues studies are small and we can talk about all the limitations right But let's just go with their conclusions There's a little bit of an impact from my functional therapy So my question is first of all in some patients there is no control because whatever you're doing voluntarily you have no control when you're asleep So that's one issue If it has an impact the impact is small The question is is it also sustainable they gotta do this myofunctional therapy for the rest of their life”

Lower incisors do NOT procline with a bone-anchored Herbst SNIPPET

The low incisor proclination This is a really interesting one Whereas the low incisor is typically a procline because an appliance is attached to them In this bone-anchored herbst the attachment is to the mandibular structure itself The low incisors do not procline Better yet the low is actually retro-cline in the process of correction which allows maximum stimulation of the skeletal structures for the overjet correction rotation of the mandible

And this is where Hugo broke down how this appliance works Typical the condyle is displaced only for the first 2 to 3 months with a conventional functional appliance The condyle is displaced the lower incisors procline As they procline the condyle reseats as the activation is reduced on the mandible Whereas with the bone anchored Herbst the fixation is on the mandible itself the condyle therefore remains displaced throughout the duration of the functional appliance therapy using the bone anchored Herbst appliance So for ten months it is left in this displaced position This causes stimulation of the mandible throughout the entirety resulting in the growth occurring
