Saw this from a while ago. So easy that all us crayon eaters can understand how quick simple and easy this to employ...


Sometimes we forget how easy it is to forget simple things. Like pressing epi or even epi drips. More services should carry phenylephrine BTW. Time saver and easy to make and manage. Lazy getting going but no worries we coming!!

High risk refusals do's and dont's
1. Vitals
2. Shift change


All to often back in the day it was common place to wack your patient with some IV bubble gum wake them from the death spiral and then it's "Sign here". Refusal documentation will keep you licensed and employeed when sh@t hit's it. Remember a few things. Vitals, mini mental, bgl, repeat bgl, making sure your patient will have someone to look out for them. Above all how do you feel about it. Should they go?? When the spider sense is tingling that's when you may want to opt for transport. Besides you'll probably be back there in 2 hour when the neighbors call to report someone face down in the driveway trying to inch worm to there phone before they go out again.

Says "LL" for a reason. Does lead placement matter?


Depends really. Are you monitoring or diagnosing. Belly leads are usually for in hospital monitoring so your patient can watch TV and move about a little without the ekg box doing the vfib dance.

Prehosptial lead placement is for diagnosing and those leads if improperly placed may give false positives / negatives of pathology. One caveat would be interfacility transport where again your monitoring 4 leads not 12 and your patient can screw around on there phone for 30 mins without doing the vfiib dance on the shock box. Let me know your thoughts??

Pop quiz!!
Your dealing with a patient complaining of ABD pain Xs infinity days. Vital signs are no one gives a s**t because it's 3am but she needs an IV. She then tells you "I'm a hard stick!!". You politely say to her "calm down, just a quick stick and a pinch" and make your way to the venous hwy. Then she pulls away violently, rips off the B/P cuff, pulse Ox and EKG leads that you had to stuff under an enormous less than hygienic breast to place properly and yells at the top of her parlament filled lungs "CALM DOWN!!" "YOU TELLIN ME TO CALM DOWN...F@ YOU, IM F$%KING OUT OF HERE!!"
DO YOU:
A. Tell her to calm down again and enjoy the show.

B. Politely apologize and try to deescalate the situation because your a professional.

C. Get up from the bench and give her the by all means bitch... bouce look.

D. Distract her while your partner draws up the ketamine to drop her like a bull elephant in the serengeti.
