For people who experience back pain with sciatica, meaning their pain radiates into their leg, it may be worthwhile to start physical therapy right away, according to new research reported October 6, 2020, in Annals of Internal Medicine. That approach is different from the initial advice doctors often give patients with back pain, which is to try to remain active and give their symptoms time to subside before considering treatment like physical therapy.

The study, a randomized clinical trial led by researchers at University of Utah Health, showed that patients with sciatica who were treated with four weeks of physical therapy experienced less disability in the year following their initial doctor's visit than patients who followed the usual wait-and-see approach.

OSU study follows 120 adolescents

For adolescents with symptoms following a concussion, starting physical therapy (PT) earlier provides outcomes similar to those of later PT, suggests a study in a recent issue of The Journal of Neurologic Physical Therapy (JNPT).

“Multimodal PT interventions administered by licensed physical therapists may be feasible and safe even within the first few weeks after injury to help facilitate prompt recovery and mitigate the onset of secondary effects from delayed treatment,” wrote Catherine Quatman-Yates, DPT, PhD, of Ohio State University. The study is part of a JNPT special issue on "Rehabilitation Management of Concussion," highlighting research-driven changes geared toward promoting return to activity in young patients with concussion.

Rehab is critical, but don’t rush it!

Moderation in the initial stages is the main message from new research that used 3D models made of a water-rich collagen gel cultured with fragments of blood vessels taken from fat. The findings indicate that rushing into rehab may hinder the growth of blood vessels.

The work, done in a collaboration of scientists from the University of Oregon’s Phil and Penny Knight Campus for Accelerating Scientific Impact, Georgia Tech and three other institutions, is detailed in Science Advances.

Next year will have quite a few changes for CMS in store

Philosophers who studied Plato as well as Hermes quoted that “the only thing consistent in this great world of ours is change”. It is known in physics and in science, that nothing stays the same, that everything is in a state of flux, meaning moving from one energy state to another. We see this all of the time in healthcare. It should come as no surprise that 2021 is posing for yet more changes

Each year near the end of July, the Centers for Medicare and Medicaid Services (CMS) releases a document with all of the proposed policy changes to be implemented in the coming year. The proposed rule for 2021 was released and included some legislative changes that are indisputable wins for PTs, OTs, and SLPs as well as some changes and payment cuts that are potentially detrimental for the entire industry.

Honor bestowed by ASAE in recent Coronavirus response piece

APTA has been named to a national list of "100 Associations That Will Save the World" in recognition of our association’s response to the COVID-19 pandemic.

The list, compiled by the American Society of Association Executives, includes APTA and 18 other associations as leaders in the "Coronavirus Response" category. In describing its list, ASAE writes, “This year has proven that we can't always predict what problems may arise,” adding that "these 100 associations, and many others, are up to the challenge.”

As an example of the APTA's work, ASAE cites an association project that linked PTs and PTAs interested in COVID-19-related health care volunteer opportunities with facilities in need in the early stages of the pandemic.

Motorium et Sensorium Unum: Part 1 Muscle Spindles

Muscle Spindles are a complex and integral part of our bodily functions; let's examine them.

Thomas Hanna (1995) in his essay, “What is Somatics” writes, “The sensory motor system functions as a ‘closed-loop feedback system’ within the soma. We cannot sense without acting and we cannot act without sensing” (p. 345). In this next series I would like to explore the details of this phenomenon by getting very granular with the requisite anatomy. In this article I will examine the relationship between muscle spindles and their anulospiral receptors. The majority of the information in this article comes from the book, “Job’s Body: A Handbook for Bodywork” by Deane Juhan.

About ten years ago, Pieter de Smidt was looking to add to his array of manual therapy skills for soft tissue treatment.

de Smidt, PT, DPT, Cert. MDT, MTC, owner of Reset-Wellness Physical Therapy in Houston, TX, was well-versed in instrument-assisted soft tissue mobilization (IASTM), which still proved painful for some patients. But after seeing the benefits of ‘distracting’ the tissues by using taping, he decided to expand that repertoire.

“Cupping seemed to be an obvious answer,” recalled de Smidt. “I figured maybe we should try distracting the tissue rather than always tearing the tissue. This was something people could tolerate.”

When someone requires abdominal surgery, there is an underlying dysfunction which needs to be corrected. The surgery to address the issue at hand is wide and varied depending on the situation. It may involve repairing a hernia or part of the digestive tract. It may involve removing an organ or removing an infant that cannot be delivered traditionally. While the primary problem may be corrected with surgery, dysfunction can continue after the fact. The new issues that can arise post-operatively after abdominal incisions can nonetheless be immediate, debilitating, and if left unchecked - chronic in nature.

In anticipation of the American Physical Therapy Association’s centennial anniversary (coming to 2021), the association recently announced the availability of “100 Milestones of Physical Therapy.”

"100 Milestones of Physical Therapy," a web-based, multimedia journey through the past 99 years of the profession, beginning with APTA's founding in the wake of World War I through the celebration of the association's centennial. The timeline touches on not just the history of the profession and the association, but the ways in which the profession affected the broader U.S. healthcare system — events such as the enactment of Medicare legislation in 1965, the signing of Americans with Disabilities Act in 1990, the elimination of the hard cap on therapy services under Medicare in 2018, and more.

While ACL injury rehabilitation programs often focus on the physical recovery of the knee and the muscle groups around it, research from Ohio University’s College of Health Sciences and Professions indicates the brain also plays a major role in successful returns after an ACL injury.

Dustin Grooms, PhD, associate professor in the School of Applied Health Sciences and Wellness, recently led the first phase of a $750,000 research grant from the United States Department of Defense with the goal of improving the orthopedic healthcare for the war fighter and civilian populations.

You may have learned that “Kegel exercise strengthens the group of muscles called the pelvic floor muscles (also known as PC muscles which stand for pubococcygeus.) These muscles contract and relax around the bladder and the bladder opening at your command. When these muscles are weak, urine leakage may result. You can exercise these muscles just like any other muscle in your body and building them up may help reduce your symptoms.
