Must knows for Antihypertensives ⚠️

✅All antihypertensive can cause ORTHOSTATIC HYPOTENSION Educate your patient to get up SLOWLY, and dangle their feet on the side of the bed before getting up

✅Do not stop antihypertensives abruptly This can cause rebound hypertension, they MUST be tapered

✅Antihypertensives are NOT a cure Educate the patient to make healthy lifestyle choices as well. These include exercise, low sodium diet, etc.

✅Always monitor heart rate & blood pressure BEFORE & AFTER administration

✅If we give the medication when the blood pressure is too low, it can be very dangerous & drop the BP further. We want to lower blood pressure but also keep it at a healthy range, avoiding severe hypotension.

✅Monitor labs, especially with potassium-sparing diuretics (AKA Spironolactone!) Hyperkalemia can lead to life-threatening arrhythmias!

-Graft vs. Fistula-

Both are used as access for hemodialysis

- Graft - Synthetic graft material is inserted between an artery and a vein Increased risk for infection due to the synthetic material insertion

- Fistula - Joining an artery to a vein Ischemic steal syndrome (Too much blood is diverted to the vein, away from the hand, causing decreased perfusion to the hand & ischemia)

- Evaluation of patency - Anywhere else, a bruit is a bad thing, but in this case it means the access is patent and ready to be used! If you do not feel a thrill or hear a bruit, this is an alarming sign and the HCP needs to be notified.
