🤓trivia
- Ang pangalan ng disease na ito ay galing sa Latin word na hordeolum meaning barley dahil mukha raw butil ng barley. Sa mga hindi alam ang hitsura ng barley, see lower left ng attached photo.

🤓causes
🖤most commonly caused by bacteria called Staphylococcus aureus

🤓signs and symptoms
🖤masakit
🖤feeling na may something na pumasok sa mata (foreign body sensation)
🖤may nana

🖤maga at mapula ang maliit na part ng eyelid

🤓diagnosis
- usually by check up and no tests

🤓treatment
- depende sa result ng check up
- many will need only warm compress at ipapatak na antibiotics
- iyong iba need oral antibiotics
- iyong iba nirerefer sa ophtha or pedia ophtha para hiwaan at madrain ang nana

reminders for my patients:

1. "SICK ONLY" - Sick means may karamdaman, may sintomas, NOT well. DO NOT BRING WELL BABIES/WELL CHILDREN on my sick child day unless may prior approval for acceptable reason.

🤓ex. of acceptable reason: urgently need vaccination and may valid reason ka kaya hindi kayo available sa ibang araw

2. Avoid messaging me on tuesdays and thursdays.

- May physical clinic ako on mondays and wednesdays. Alam niyong wala akong tuesdays and thursdays kaya dapat pinacheck niyo na iyan earlier the day before.
- Bukas ang ER 24/7 kung REAL emergencies.

- Wala akong nakikitang sense na madaliin niyong makita ko ang patient on tuesdays dahil late natatapos ang clinic ko on mondays and ang aga ko magstart sa wednesdays. Ang dami niyong chance to see me sa normal na oras ko.
- On thursdays, yes I understand your concern... medyo mahaba ang interval between end of my wednesday clinic and start of my friday clinic. That is why may thursday 9-10 AM ako dati sa Hipre kaso they refuse to allow me to continue that sched. Anyway, if totoong urgent iyan meaning may acceptable reason para maniwalang hindi na makakapaghintay ng friday iyang concern, message my secretary [ma'am Fe] sa Pope for appointment. If you don't know here number, call the hospital [+639060585109]. 4-5 PM meaning 4 or earlier kayo darating and 5 or earlier ako aalis HINDI 5 on the way pa lang kayo. Ang mga magpapa-appointment na hindi sisipot will be banned forever.

FYI antibiotics ang clindamycin. Ito rin ang antibiotics na ginagamit sa matatapang na impeksyon sa balat. Kung nasanay ang bacteria sa matapang na antibiotics, ano ang gagawin? Eh di iaadmit kayo para turukan ng even more matapang na antibiotics.

reminder:

Kapag lumabas ang symptoms* ng rabies, ang chance na mamamatay ang pasyente ay 100%. Rabies is preventable but not curable. I understand na gusto niyo magpahinga pero magiging forever ang rest ng pasyente kapag hindi pina-inject dahil lang nakabakasyon. Prioritize rabies vaccination hindi iyong kung ano ano ang dahilan.

Isa lang ang buhay ng anak niyo. Ayusin po ang pagdedesisyon.

*There is no such thing as mild rabies or mild symptoms of rabies. Basta nagkasymptom automatic death ang ending. Hindi siya something na maaagapan pa kapag dinala na "mild pa". Ang injections ay after exposure but BEFORE symptoms.

Bakasyon mode muna ang utak ko so no medical post muna. Share lang ako ng random trivia about me 😄. I am one of the very few doctors na HINDI mahilig sa kape. Almost all doctors I know can't function without coffee.

Once in a while lang ako umiiinom kapag:
1. nasa lugar na famous for coffee
ex. Itong nasa photo ay binili ko during my Vietnam trip kasi sikat sila sa coffee. Majority ng binili ko pinamigay ko rin 😅 pero nag-iwan ako konti to try them.
2. binilhan ako ng pasyente or med rep
3. libre ang kape at gusto ko matry

Why?
- nagpapalpitate ako
- Hindi ko nagugustuhan ang lasa ng most coffee.
- Ayoko maging dependent sa caffeine. I survived med school and residency without caffeine by using certain study techniques so walang reason na ngayon pa ako magkape kape.

🤔"Kailan ba nirerefer ang pasyenteng may hika?"

🤓Based sa 2024 edition ng libro namin:
🤓For children ≤4 years, referral is recommended if the patient requires at least treatment step 3 care, and should be considered if the patient requires Treatment Step 2 care. [meaning usually iyong mga naka-Seretide na]
🤓For children ≥5 years, consultation with a specialist is recommended if the patient requires Treatment Step 4 care or higher, and should be considered if treatment step 3 is required. [usually mga hindi na ma control even on max dose of pambatang Seretide... sometimes we try to add montelukast pa muna...]

👨🏻‍⚕️Ito ay general guide lang at hindi absolute rules. Kailangan din naming iconsider ang things like budget ng magulang, availability ng needed subspecialist/s, etc.
👨🏻‍⚕️Sa mga nirefer, sometimes two brains ay better than one so please sumunod kayo for best outcome.
👨🏻‍⚕️Sa mga marereferan, refer back kung hindi naman transfer service ang reason for referral. Hindi na kayo marereferan kung hindi niyo binabalik ang mga pasyente.

LETHARGY

Ang ibig sabihin ng lethargy antok na antok ang bata at kailangan pa ng malakas na boses bago siya magigising... iyong antok na lagpas na sa usual sa pasyente... antok na lagpas sa expected sa haba ng tinulog na niya (ang haba na ng tulog pero sobrang antok pa rin).

Marami pong possible causes ito like mababa na ang blood sugar dahil hindi dumedede o kumakain, malala na ang infection, meningitis, encephalitis, abnormal na ang level ng asin sa dugo, etc.

Kahit hindi pa matagal ang ibang mga sintomas ng bata pero kapag biglang nasabayan na ng lethargy, then pacheck up na agad. Baka kailangan nang i-confine ang bata. Hindi naman 100% ay iaadmit pero doon na tayo sa anong mas safe. Mas okay na ang OA sa pacheck up kaysa puro observe sa bahay tapos malala na pala.

Wala akong nakaplanong lakad outside Las Pinas so normal clinic hours po tayo until otherwise announced. Unless may pinost akong changes, automatic may clinic on all my normal clinic days clinic hours. Hindi ako nakikisabay sa pag-alis ng ibang mga pedia dahil kailangang may maiwan to take care of sick children. Hindi naman namimili ng panahon ang mga sakit. Next year na siguro ako gagala.

Malagkit na Gamot

🤔"Ang hirap ipainom ng gamot sobrang lapot. Ano ba dapat gawin? "

🤓1. Check

Sure bang right amount of water ang nalagay? Mukha namang match ang total dami ng gamot sa mL na indicated sa bote?
🖤Kung kulang ang water siyempre magiging unusually malapot iyan.

Nahalo naman maigi ang gamot?
🖤Baka naman nalimutang ishake tapos more of water iyong mga naunang dose tapos concentrated gamot iyong naiwan.

🤓2. Use syringe to measure.

🤓3. Dilute with non-sticky fluid if approved by pediatrician.

ex. diluting 5 mL cefuroxime with 15 mL fruit juice

🤓4. Ilayo sa dila ang gamot by using straw or syringe para hindi mag-stick sa dila.

🤓5. Sundan agad ng fluid para hindi tumagal sa bibig ang gamot.

🤓6. Bagalan.

ex. Ang babies ilang ounces ng gatas kaya nang halos walang hintuan pero look at how slowly pediatricians give rotavirus vaccine.

👨🏻‍⚕Maraming klase ng gamot at bata so what works for one patient may not work for another. Mix and match techniques to achieve successful na pagpapainom.
👨🏻‍⚕We can't always give medicines na madali ipainom. You have to learn to give sticky na gamot also.
