文件準備
1. 疫苗施打紀錄（建議使用有出生日期之版本）
2. 加拿大本地電話號碼
3. 可正常使用的Email
4. 多倫多住處郵政編碼 （Postal Code）
步驟
1. 前往 Toronto Public Health網站
Out of Province Dose Documentation (chkmkt.com)
2. Please enter your City of Toronto postal code
3. Read the Acknowledgement of Collection
8. 致電 1-833-943-3900
成功接駁到Officer後，他會向你確認一下資料：
FIRST NAME
LAST NAME
SECOND DOSE DATE
DATE OF BIRTH
確認後會轉接到另外一個部門，之後會再確認一次所有資料
另外你需要提供電話號碼 & 之前使用的Email
9. Officer確認資料無誤後會給你發送一封電郵
10. 打開電郵內的鏈接
11. 輸入第二劑施打時間以及出生日期
12. 下載你的 Enhanced Vaccine Certificate
13. 如果你使用Iphone，可以直接用Camera 拍攝二維碼
並把疫苗紀錄添加至Apple Health
